This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a request for quotations (RFQ). The quote document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-79 December 15, 2014. This RFQ is Restricted to Small Businesses. The associated North American Industrial Classification System (NAICS) code for this procurement is 624310, with a small business size standard of $11,000,000.00 in Annual Receipts. The Network Contracting Office 8 is seeking a contractor that shall provide instruction on Job Seeking Skills in a classroom setting on-site at the C.W. Bill Young VA Medical Center, Bay Pines, Florida in accordance with the Statement of Work (SOW) below. All questions regarding this RFQ must be in writing and may be sent by e?mail to steven.scharlow@va.gov. Questions must be received no later than 3/19/2015 at 9:00 AM. No further questions will be accepted after that date and time. Answers to vendor questions will be posted on FBO no later than 3/20/2015 at 4:30 PM EST. All questions and concerns regarding this RFQ shall be directed to the Contracting Officer. QUOTE FORMAT AND EVALUATION CRITERIA 1.Offerors shall complete Attachment A CLIN Pricing. Not completing and returning attachment A CLIN Pricing will result in disqualification. Price will be ranked based on the lowest summary price to include all options. 2.Technical Requirements that the Contractor Shall Provide: a.Contractor to provide evidence that the instructor(s) have a minimum of 3 - 5 years of experience in teaching job skills training/computer education. This can be in the form of a personal resume containing work experience and references. (Go/No Go) b.Provide a training outline/agenda showing that they will provide the training identified in the Statement of Work. (Go/No Go) 3.Offerors shall provide up to three references of their performance providing the same or similar services matching the scope of this requirement within the last three years having the same magnitude and complexity as this requirement. Disputes (July 2002) (a) This contract is subject to the Contract Disputes Act of 1978, as amended (41 U.S.C. 601-613). (b) Except as provided in the Act, all disputes arising under or relating to this contract shall be resolved under this clause. (c) "Claim," as used in this clause, means a written demand or written assertion by one of the contracting parties seeking, as a matter of right, the payment of money in a sum certain, the adjustment or interpretation of contract terms, or other relief arising under or relating to this contract. However, a written demand or written assertion by the Contractor seeking the payment of money exceeding $100,000 is not a claim under the Act until certified. A voucher, invoice, or other routine request for payment that is not in dispute when submitted is not a claim under the Act. The submission may be converted to a claim under the Act, by complying with the submission and certification requirements of this clause, if it is disputed either as to liability or amount or is not acted upon in a reasonable time. (d)(1) A claim by the Contractor shall be made in writing and, unless otherwise stated in this contract, submitted within 6 years after accrual of the claim to the Contracting Officer for a written decision. A claim by the Government against the Contractor shall be subject to a written decision by the Contracting Officer. (2)(i) The Contractor shall provide the certification specified in paragraph (d)(2)(iii) of this clause when submitting any claim exceeding $100,000. (ii) The certification requirement does not apply to issues in controversy that have not been submitted as all or part of a claim. (h) The Government shall pay interest on the amount found due and unpaid from (1) the date that the Contracting Officer receives the claim (certified, if required); or (2) the date that payment otherwise would be due, if that date is later, until the date of payment. With regard to claims having defective certifications, as defined in FAR 33.201, interest shall be paid from the date that the Contracting Officer initially receives the claim. Simple interest on claims shall be paid at the rate, fixed by the Secretary of the Treasury as provided in the Act, which is applicable to the period during which the Contracting Officer receives the claim and then at the rate applicable for each 6-month period as fixed by the Treasury Secretary during the pendency of the claim. Title of Project: Job Skills Training Program 2. Scope of Work: Contractor shall provide instruction on Job Seeking Skills in a classroom setting on-site at the C.W. Bill Young VA Medical Center, Bay Pines, Florida. All curriculum shall be designed to promote and educate Veterans on how to manage mental wellness and acquire the job related skills needed to succeed in a competitive work environment. Each 3 hour class will focus on a specific skill set required to experience success in pursuing vocational goals related to work. Class format will consist of the utilization of written handouts, verbal presentation of the content of subject matter, video on subject matter, written exercises, and support from instructor and peers in gaining knowledge. 3. Background: The Bay Pines VAHCS requires access to specialty clinics to supply Employment Re-Entry Services (ERS) to unemployed Veterans. Unemployment often results in an exacerbation of a multitude of mental health symptoms, substance disorders, financial hardship, and homelessness. Veterans require timely access to vocational assistance in the pursuit of goals of competitive employment. For this reason it is beneficial to the Government to have a vendor available who can provide a variety of specialized Job Skills Training to increase Veteran employment rates. 4. Performance Period: The vendor shall begin to provide the services requested under this SOW immediately following the award of the contract, unless otherwise directed by the Contracting Officer (CO). The contract will extend for one year from date of award (Base Year); at which time, 4 option years may be utilized at the sole discretion of the Government. Classes will be provided throughout the year (excluding holidays observed by the Federal Government). The ten holidays observed by the Federal Government are New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. Also, any other day declared by the President of the United States to be a national holiday. 5. Type of Contract: This contract is a Firm Fixed Contract (FFP). B. CONTRACT AWARD MEETING The vendor shall not commence performance on the tasks in this SOW until the CO has conducted a kick off meeting, or has advised the vendor that a kick off meeting is waived. C. GENERAL REQUIREMENTS 1. Immediately following the award of the contract vendor shall provide Veterans engaging in ERS with a viable job skills training curriculum that instills knowledge of transferable work skills for veterans engaged in competitive employment. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: Task One: The vendor shall provide job skills training for up to 15 students when in the computer lab and up to 20 when in the classroom setting, (3) hours a day, (4) times a week, in (5) week increments, for a total of (624) hours annually. The Vendor shall provide continual classes throughout the year. Occasionally, in the event that scheduled classes fall on Federal holidays the VA will work with the contractor to schedule make-up days. Deliverable One: A detailed curriculum should include, but is not limited to "Instructor - Instructor shall have a minimum of 3 - 5 years of experience in teaching job skills training/computer education. "Communication skills - Focuses on topics of effective communication types of interpersonal communication, nonverbal communication, components of good communication, clear and concise speech patterns, listening to understand, levels of listening, key components of effective listening, and definition of and review of behavior styles, voice quality, and telephone communication skills. "Personal Assessment and Image Awareness - Covering professional image, personal presence, making first and lasting impressions, completion of a positive self- image test and characteristic matrix, decorum, time management, goal setting, image awareness via positive self-image, professionalism, and professional responsibilities. "Interviewing Techniques - Key elements of job interviews, interview etiquette tips, work sheets related to interviews, review of difficult interview questions, what to wear on a job interview, as well as completion of a mock interview with educator "Goal setting for success - Instruction, and education surrounding use of the "S.M.A.R.T" principle when identifying personal and professional goals, ten steps of goal setting, four principles of goal setting, setting and achieving goals, development of a personal action plan for setting and achieving goals, benefits of goal setting, how to realistically evaluate progress towards/completion of goals, and work sheets related to establishing goals for oneself. Students will receive classroom education and computer lab experience practicing the skills learned. E.SCHEDULE FOR DELIVERABLES 1. The contractor shall deliver first class within 2 weeks of notification of Award and weekly thereafter. 2. The contractor shall provide 24-hour notification if the instructor is ill or cannot perform expected educational training. Contractor will identify a replacement for instructor within a 2-week period, should the instructor be unable to perform agreed ongoing educational training. 3. If for any reason any agreed upon services cannot be delivered within two-month duration from date of notification, the contractor is required to explain why they are unable to fulfill contractual obligation in writing to the Contracting Officer (CO). This notice to the CO shall cite the reasons for the delay, and how the delay will be avoided in the future. The CO will then review the facts and issue a response, in accordance with applicable regulations. F. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. G. REPORTING REQUIREMENTS 1. Quality assurance program and performance measures; Participants shall complete a Satisfaction survey upon completion of the program. The data is analyzed by Quality Systems and reported to the Employment Re-entry Services. 2. The contractor will submit invoices on a monthly basis to the identified Contracting Officers Representative (COR) for review and submission to the Austin Finance Center for payment. H. GOVERNMENT RESPONSIBILITIES [If applicable] Government agrees to provide contractor with classroom space with tables and chairs sufficient for class size; computer lab that is run as an independent network from VA network; white board and/or smart board; Television, projector, and DVD players. I. CONTRACTOR EXPERIENCE REQUIREMENTS Instructor shall have a minimum of 3 - 5 years of experience in teaching job skills training/computer education. The government will determine on a SR basis what positions are considered key personnel. The contractor shall identify, by name, the key management and technical personnel who will work under this task order at the time the work is being negotiated. If a key person becomes unavailable to complete the SR, the proposed Substitutions of key personnel shall be made only as approved directed by the Contracting Officer and the COR. The government will not dictate specific experience and education requirements of the employees initially proposed to perform the work stated herein. The contractor shall submit a resume of qualifications for the COR and all other direct employees proposed for the project. All Contractor employees will be approved by the COR prior to bringing on duty. If, at any time from date of award to the end of the contract, non-key personnel Contractor personnel are no longer available, HES/HIS reserves the right to review the qualifications of the proposed replacement personnel and to reject individuals who do not meet the qualifications set forth in the TLO. Team personnel proposed by the contractor should possess some of the following knowledge and/or skills: The contractor must notify VHA in advance and we will approve or reject proposed contractor key personnel for the performance of this contract. The contractor shall submit a resume of qualifications to the COR for key personnel and all other direct employees proposed for the project. All Contractor employees will be approved by the COR prior to bringing on duty. If, at any time from date of award to the end of the contract, Contractor personnel are no longer available, the VHA will approve the qualifications of proposed replacement personnel and will reject individuals who do not meet qualifications set forth herein. The contractor must inform the VHA COR, and Project Manager/Task Manager when personnel are removed from the contract for any reason. The Contractor shall remove any employee from the performance of this contract within five (5) workdays of receiving notice from the Contracting Officer that the employee's performance is unsatisfactory. All Contractor employees are subject to immediate removal from performance of this contract when they are involved in a violation of the law, VA security, confidentiality requirements and/or other disciplinary reasons. "The contractor shall submit a resume of qualifications to the COR for all direct employees proposed for the project. All Contractor employees will be approved by the COR prior to bringing on duty. L. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO.
