Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation only; Request for quotations are being requested & a written solicitation will not be issued. The solicitation number is 246-2015-Q-0011 and is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-78. This procurement is being conducted on an unrestricted basis. All eligible businesses may submit a quote, which will be considered. This solicitation is for Non-Personal Service of mammography equipment maintenance and service to be utilized at the Pawnee Indian Health Center in Pawnee, Oklahoma. Specifically, the requirement is for the Hologic Selenia 2D Dimension and R2 CAD 1st Port w/DigitalNow - LS wide feeder. The anticipated period performance is April 1st, 2015 through March 31, 2016. Offerors are to include a completed copy of the provision found at FAR 52.212-3, Offeror Representations & Certifications - Commercial Items with its offer. The clause found at FAR 52.212-4, Contract Terms & Conditions Required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition.
