SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Philadelphia VA Medical Center, University & Woodland Ave. Philadelphia, PA 19104 is conducting a market survey to identify potential sources to provide maintenance services of Reverse Osmosis Hemodialysis Equipment. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing maintenance services of Reverse Osmosis Hemodialysis Equipment located at the Philadelphia VA Medical Center, which includes the services of skilled technical personnel and necessary equipment and supplies. The equipment currently housed at the Philadelphia VA Medical Center is a Mar Cor Reverse Osmosis Machine, Model No. WRO104H28LE, The North American Industry Classification System Code (NAICS) is 811219 and the small business size standard is $19.0 million. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of maintenance services of Hemodialysis Equipment. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 811219 and the small business size standard is $19.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.Provide a summary of Maintenance services performed on hemodialysis equipment. 4.Provide a summary as to whether or not the maintenance services include all necessary supplies (i.e., chemicals, carbon filters, chemical disinfection, etc.). Include a list of what is provided and their function as said supplies relate to the maintenance services. Include a reference as to whether a MSDS is required and can be provided should a contract be awarded. 5.Equipment water Quality must be maintained in accordance with the Association for Advancement of Medical Instrumentation (AAMI) standards. Provide a summary of how maintenance services meet these standards, to include frequency of maintenance visits needed. 6.Should there be a malfunction or breakdown in the equipment, please provide a description of what repair services may be available and the minimum turnaround time in which service can be restored In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
