Description
COMBINED SYNOPSIS/SOLICITATION - This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a Request for Quote (RFQ). Submit a written quote on RFQ reference number VA249-15-Q-0368. The Government requires that all Contractors doing business with this office be registered with System for Award Management (SAM). No award can be made to a company not registered in SAM. For additional information and to register in SAM, please access the following web site: https://www.sam.gov/. Please ensure that the representations and certifications are completed to allow Contracting Officers to be able to determine your business size and any additional socio economic categories, if applicable. (iv) This solicitation is unrestricted, the associated NAICS code is 517110 with a small business size standard of 1500 employees. (v) This combined synopsis/solicitation is for the following commercial service: CLIN 0001- The Contractor shall provide all labor, equipment, facilities, tools, materials, supervision, travel, shipping and/or services necessary to provide terrestrial or satellite television service for the James H Quillen VA Medical Center for approximately 782 televisions as per attached Statement of Work. BASE YEAR: 1 April 2015- 31 March 2016: $_____month X 12 months = $_______________. OPTION YEAR ONE: 1 April 2016- 31 March 2017: $_____month X 12 months = $_______________. OPTION YEAR TWO: 1 April 2017- 31 March 2018: $_____month X 12 months = $_______________. OPTION YEAR THREE: 1 April 2018- 31 March 2019: $_____month X 12 months = $_____________. OPTION YEAR FOUR: 1 April 2019- 31 March 2020: $_____month X 12 months = $______________. (vi) A description of the requirements is as follows: The Mountain Home Veteran Affairs Medical Center has a requirement for television service for approximately 782 televisions located in the patient waiting areas and various patient rooms. Please see Statement of Work for detailed information. (vii) Anticipated Period of Performance is 1 April 2015 through March 31, 2016 for the base period. Performance will take place at the James H. Quillen VA Medical Center, Mountain Home, TN. (viii) FAR 52.212-1, Instructions to Offerors - Commercial Items (Jun 2008), applies to this acquisition. Addenda: Offers may be submitted in any format that the offeror desires as long as it contains all the required information contained in section b of this provision. (ix) FAR 52.212-2, Evaluation -- Commercial Items (Jan 1999), applies to this acquisition. (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1.Price, 2.Technical, LOWEST PRICE TECHNICALLY ACCEPTABLE In accordance with FAR 15.101-2, Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. TECHNICAL ACCEPTABLE/UNACCEPTABLE RATING The Contractor shall submit documentation that demonstrates qualifications the channel line-up they are offering as per the Statement of Work. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). Quillen VA Medical Center Mountain Home, Tennessee 1. GENERAL SCOPE OF WORK: Background: The Mountain Home VAMC (Veteran Affairs Medical Center) serves veterans in the Johnson City, Tennessee and surrounding Eastern TN area. The VAMC is a 24 Hour/365 day facility and has several large reception/waiting areas for the veterans and their families to relax and await their appointments. The waiting areas have wall mounted T.V's for viewing by the veterans and their families. There are also several inpatient wards that also have wall mounted televisions in each patient room. Description: Contractor shall provide all supervision personnel, equipment, transportation, Material and other items and services necessary to provide Satellite or Terrestrial television service for the James H Quillen VA Medical center, Mountain Home TN for the period beginning April 1, 2015 through March 31, 2016 with four 1 year option periods in accordance with the scope of work. The service is to include all necessary equipment and supplies required to provide uninterrupted coverage of the following channels for all existing T.V. monitors (approximately 782 T.V's). oThe contractor shall make available the local service channel lineup consisting of the following local channels: CBS, NBC, ABC, FOX, FOX News, PBS, WGN, CSPAN, ESPN, AMC, TNT, TNN, USA, CNN and A&E. oThe contractor is to propose a variety and a minimum of 55 channels at the best possible rate. If alternate packages will result in a lower cost, those packages should be presented as well. The VA expects hotel/hospitality pricing to be presented where applicable with content provider listed. oIn the Channel lineup the contractor shall provide a solution to watch local sport events and S.E.C football sporting events oContractor shall provide uninterrupted Television services 24/ hours a day/ 365 days per year. oThe Contractors package shall be capable of receiving individual channels, generating video and audio output, modulated and interfaced to the existing head in hook ups for distribution to the system network. The existing head-in equipment is located in building 8. oThe subscribed TV reception shall be continuous, non-interrupted, with good viewing quality (as is congruous with industry standards). oTechnical support by telephone shall be provided during regular business hours Monday through Friday. Contractors' equipment (if required) shall be serviced on a regular schedule according to manufacturer's guidelines. Defective contractors equipment shall be repaired or replaced within 48 hours from the time of notification. oThe Contracting officers representative may require the contractor to meet with VA representatives on an as needed basis. oThe contractor shall provide accurate T.V. Channel Listings for the current channel lineup. Notes: oThe VA currently provides VA Specific television channels via notch filters and channel insertion devices on channels 70, 71, 72, and 73. The proposed television package shall not interfere with these services. oThe VA recently upgraded the cable system infrastructure. The contractor's proposal shall not degrade the channel signal strength of this system. RELIABILITY oThe Contractor shall be responsible for maintaining the operation of the hardware and all infrastructure up to the building 8 Demark location. oThe Contractor shall provide service availability with an expected 98% uptime. oThe Contractor shall provide the VA COR a singular point of contact capable of managing any issues associated with Television services, from service calls to payment and invoicing. B.1 WORK REQUIREMENTS oAll work is subject to inspection by VA COR. Work not meeting reasonable quality and craftsmanship shall be performed again at the contractor's expense. oAll work to be completed in a craftsmanship and safe manner. oAll work must meet NFPA 99 2005 code requirements. oAll work to be completed in a reasonable time frame. STATEMENT OF WORK PART C - SUPPORTING INFORMATION C.1 Place of Performance - VA Medical Center, Mountain Home, TN C.2 Period of Performance -April 1, 2015 through March 31, 2016. It is preferred that work be accomplished during regular business hours 7a.m. to 4:30 p.m. Monday through Friday if possible with evening, night, and weekend work as required C.3 Special Considerations - Contractor must adhere to all OSHA and VA Medical Center construction safety policies and standards. Contractor's employees must view a short infection control video. Contractor's employees must sign in and out using a daily log provided by the VA. The log is located in Building 37. Contractor shall ensure that all firewall and smoke partition breaches, penetrations and intrusions are properly sealed to meet NFPA 99 codes and standards. C.3.1 Contractor Furnished Materials - All labor, materials and tools to complete the work identified. All equipment, material, and articles incorporated into the work covered by this contract shall be new and less than one year old. C.3.2 Government Furnished Materials and Services: N/A C.3.3 Qualifications of Key Personnel - Construction contractor supervisor will have a 30 hour OSHA training certificate. All other personnel will have 10 hour OSHA training certificate. C.3.4 Security Requirements -Contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. Those employees shall bring photo ID to obtain a VA Contractor identification badge that shall be worn above the belt at all times while on Medical Center property. Contractor's employees shall return all I.D. badges to designated personnel at time designated in the task order. C.3.5 The Contracting Officer's Technical Representative (COR) is responsible for the inspection of the work. The Contracting Officer's Technical Representative will be identified by name, telephone number and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities. C.3.6 Smoking Policy. The Contractor shall not allow smoking in any building. The Contractor shall ensure employees smoke only in designated outdoor areas, 50 feet away from buildings. C.3.7 Parking/Traffic Regulations. Parking Permit Request form must be completed if required. The Contractor shall not park on grassy areas unless approved by the COR and the Contractor agrees to restore areas back to VA standards. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the VA grounds. MATERIAL AND WORKMANSHIP (APR 1984) (a) All equipment, material, and articles incorporated into the work covered by this contract shall be new and/or of the most suitable grade for the purpose intended, unless otherwise specifically provided in this contract. References in the specifications to equipment, material, articles, or patented processes by trade name, make, or catalog number, shall be regarded as establishing a standard of quality and shall not be construed as limiting competition. The Contractor may, at its option, use any equipment, material, article, or process that, in the judgment of the Contracting Officer, is equal to that named in the specifications, unless otherwise specifically provided in this contract. (b) The Contractor shall obtain the Contracting Officer's approval of the machinery and mechanical and other equipment to be incorporated into the work. When requesting approval, the Contractor shall furnish to the Contracting Officer the name of the manufacturer, the model number, and other information concerning the performance, capacity, nature, and rating of the machinery and mechanical and other equipment. When required by this contract or by the Contracting Officer, the Contractor shall also obtain the Contracting Officer's approval of the material or articles which the Contractor contemplates incorporating into the work. When requesting approval, the Contractor shall provide full information concerning the material or articles. When directed to do so, the Contractor shall submit samples for approval at the Contractor's expense, with all shipping charges prepaid. Machinery, equipment, material, and articles that do not have the required approval shall be installed or used at the risk of subsequent rejection. (c) All work under this contract shall be performed in a skillful and workmanlike manner. The Contracting Officer may require, in writing, that the Contractor remove from the work any employee the Contracting Officer deems incompetent, careless, or otherwise objectionable. (End of clause) PERMITS AND RESPONSIBILITIES (NOV 1991) The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any Federal, State, and municipal laws, codes, and regulations applicable to the performance of the work. The Contractor shall also be responsible for all damages to persons or property that occur as a result of the Contractor's fault or negligence. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract. (End of clause) PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND IMPROVEMENTS (APR 1984) (a) The Contractor shall preserve and protect all structures, equipment, and vegetation (such as trees, shrubs, and grass) on or adjacent to the work site, which are not to be removed and which do not unreasonably interfere with the work required under this contract. The Contractor shall only remove trees when specifically authorized to do so, and shall avoid damaging vegetation that will remain in place. If any limbs or branches of trees are broken during contract performance, or by the careless operation of equipment, or by workmen, the Contractor shall trim those limbs or branches with a clean cut and paint the cut with a tree-pruning compound as directed by the Contracting Officer. (b) The Contractor shall protect from damage all existing improvements and utilities (1) at or near the work site, and (2) on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor. (End of clause) CLEANING UP (APR 1984) The Contractor shall at all times keep the work area, including storage areas, free from accumulations of waste materials. Before completing the work, the Contractor shall remove from the work and premises any rubbish, tools, scaffolding, equipment, and materials that are not the property of the Government. Upon completing the work, the Contractor shall leave the work area in a clean, neat, and orderly condition satisfactory to the Contracting Officer or the Contracting Officer's Technical Representative (COR). (End of clause) ACCIDENT PREVENTION (NOV 1991) (a) The Contractor shall provide and maintain work environments and procedures which will- (1) Safeguard the public and Government personnel, property, materials, supplies, and equipment exposed to Contractor operations and activities; (2) Avoid interruptions of Government operations and delays in project completion dates; and (3) Control costs in the performance of this contract. (b) For these purposes on contracts for construction or dismantling, demolition, or removal of improvements, the Contractor shall- (1) Provide appropriate safety barricades, signs, and signal lights; (2) Comply with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR Part 1910; and (3) Ensure that any additional measures the Contracting Officer determines to be reasonably necessary for the purposes are taken.. (d) Whenever the Contracting Officer becomes aware of any noncompliance with these requirements or any condition which poses a serious or imminent danger to the health or safety of the public or Government personnel, the Contracting Officer shall notify the Contractor orally, with written confirmation, and request immediate initiation of corrective action. This notice, when delivered to the Contractor or the Contractor's representative at the work site, shall be deemed sufficient notice of the noncompliance and that corrective action is required. After receiving the notice, the Contractor shall immediately take corrective action. If the Contractor fails or refuses to promptly take corrective action, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. The Contractor shall not be entitled to any equitable adjustment of the contract price or extension of the performance schedule on any stop work order issued under this clause. (e) The Contractor shall insert this clause, including this paragraph (e), with appropriate changes in the designation of the parties, in subcontracts. (End of clause)
