- AMENDMENT: The following text is added: There will be a sight visit on Monday, March 23, 2015 at 2:00 pm. The sight visit is expected to last 30 minutes to an hour. Any vendor interested in the sight visit will meet at the VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI 48105. We will meet at the new emergency room located at the back of the hospital. There is parking in the ER Parking lot off of Fuller Court. There is a waiting area right inside the door where everyone involved will meet. Each vendor is responsible to bring their own copy of the combined synopsis solicitation. The solicitation will also be extended through Friday, March 27, 2015 at 12:00 p.m. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, and is being conducted under FAR Subpart 13; simplified acquisition procedures apply. VA will conduct the procurement according to a combination of FAR Part 12, Acquisition of Commercial Items and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. (ii) The solicitation number is VA251-15-Q-0236 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-69, August 1, 2013. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as a small business set aside. The North American Industry Classification System (NAICS) code is 238350, and the size standard is $15 Million. DESCRIPTION/SPECIFICATIONS/WORK STATEMENT I.SCOPE OF WORK: The Contractor shall provide labor, material and supervision to remove old automatic doors and install a new automatic door located in the Emergency Room Lobby within the Ann Arbor VA Medical Center. Location for the install will be Ann Arbor VA Medical Center, 2515 Fuller Road, Ann Arbor, MI 48105. This contract is for removal of existing aluminum glass doors. The contractor will furnish and install break metal to cover up the holes on the door frame from removing the doors. The contractor will furnish and install aluminum tubing against the existing framing to bring the slider out to face the existing knee wall. The contractor will remove from the site the existing doors and any materials that come with the removal process. The contractor will then install a record-usa 4500 series or like model surface mounted bi-part slider. The installed unit will be a record-usa 4500 series or like model with surface mounted IXIO sensors, narrow stile, muntin bars, no threshold, 1 3/4" X 4 ½" frame, 4 ½" X 7" header, microprocessor controls, activation per ANSI 156.10, guide rails, ¼" clear tempered glass with a clear anodized finish. The contractor will finish the work with caulking the door. The contract will include all system programming, testing and final adjustments. II.CONFORMANCE STANDARDS: All services provided under this contract must be performed in conformance with the Federal regulations and AAVA policies and requirements. III.PREVENTIVE MAINTENANCE: All preventive maintenance will be done by the contractor through a two (2) year product warranty. IV.REPORTING REQUIREMENTS: The Contractor shall be required to report to the Carpentry Section of Facilities Management at the Ann Arbor VA Medical Center to log in during normal working hours. This check in is mandatory. When the service is complete, the Contractor shall document services rendered on a legible field service report. The Contractor shall log out at the Carpentry Section of Facilities Management and submit the field service report to the Contracting Officer Technical Representative for a signature, during normal working hours. If the Contracting Officer Technical Representative is unavailable, a signed authorized copy of the field service report shall be sent to the Contractor after the work has been reviewed. V.SAFETY REQUIREMENTS: In the performance of this contract, the Contractor shall take such safety precautions as the Contracting Officer may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer shall notify the Contractor of any safety issues and the action necessary to correct these issues. Such notice, when served on the Contractor or his representative at the work site shall be deemed sufficient for the corrective actions to be taken. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work, and hold the Contractor in default.
