- Pricing Schedule This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Chinle Comprehensive Health Care Facility (CCHCF), Navajo Area Indian Health Services is requesting for submittal of offers for a contractor to provide an ultrasound machine to the Emergency Room Department. Per FAR 6.302-1(c)(2), CCHCF has identified a specific brand name, but will accept brand-name or equal descriptions. Refer to attached Fee Schedule. EVALUATION FACTORS The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: Price, Features, and Delivery. The socio-economic status of an offeror may also be considered, should one or more quotes represent the best value. SUBMITTAL Offers shall be emailed to tanya.begay2@ihs.gov by the due date specified. Please limit offers to 3GB. To be considered for award, please complete, sign and submit the following documents: -Pricing Schedule (attached and must be signed) Interested parties are requested to submit a quote, along with all completed supporting documents as soon as possible keeping in mind the RFQ close date.
