BRAND NAME OR EQUAL ZEISS CIRRUSS OPTICAL COHERENCE TOMAGRAPHY (OCT 5000) 548-15-2-440-0045 Please provide us with a quote based on the specifications/scope of any products/services listed below. Please quote your products/services whose salient features meet or exceed the specifications/scope of the requested products/services. Any brand name and/or model/product numbers which may be indicated below are for informational purposes only and are not intended to be restrictive of competition. The indicated product number/numbers may be those of a suggested vendor (Carl Zeiss Meditec Inc.) as opposed to a manufacturer's product number. Please indicate if your offered products are offered under a FSS contract and provide the contract number and expiration date. If your products are offered open market, please indicate same on your offering.Please respond to this RFQ by email to me only or we may have to disqualify your offer. Evaluation - Please note that it is the responsibility of the respondents to this RFQ to highlight the salient features of its offered products/services if you are indicating that the offered products salient features meet or exceed the specifications of the requested product. If the descriptive information is not included or inadequate, the offer may be determined to be non-responsive to the RFQ. Our intentions is to buy and replace our existing equipment as a trade-in for our Cirrus 4000 EE# 42879 with S/N: 4000-2902, and a $25,000.00 trade in allowance. Purpose for trade-in: The current Cirrus OCT 4000 is out of date. The old machine is becoming worn out and unreliable. The equipment is used daily in the eye clinic at least 50 times a day. This machine is called Zeiss Cirrus Optical Coherence Tomography (OCT 5000). Zeiss item number: 266002-1150-2263- 1 each. Trade-In:- 1 each. And operators, service, maintenance manual- 1 each. The West Palm Beach Medical Center located at 7305 N Military Trail West Palm Beach, FL 33410, intends to issue a Request For Quote (RFQ) for (Carl Zeiss Meditec Cirrus OCT 5000) for the Eye Clinic. Type of contract will be Firm Fixed Price. This procurement is being solicited as small business set-aside the applicable NAICS code is (339115). Solicitation will be electronically issued in commercial item format in accordance with FAR Part 13 on or about (03/20/2015 on FBO as Brand Name or Equal) with an estimated offer due date of (03/25/2015)@5PM EST. Any future amendments and other miscellaneous documents will be available electronically for download at the following URL: http://www.fedbizopps.gov. Proposals are due on or about (04/03/2015)@5PM Eastern Time. The report must also show volume of drusen and provide automatic mathematical evaluation analysis that provides serial analysis with percentage of change listed. 2.Guided Progression Analysis that shows a forecast of what patients' cup-to-disc ration measurement will be in the future years on a linear graph based on the amount of change with a specified amount of time. 3.Must have compatibility with our current optical coherence tomography. 4.This piece of equipment MUST meet our requested specifications because it has FDA approved normative data bases. For optimal resolution, we are requesting that the equipment scan at least 68,000 A scans per second. 5.It must include a retinal tracking system with precise macular thickness analyses with detailed layer maps and more than 100 B-scans at our disposal to assess the patient's retinal condition. The data cubes must register with data from prior visits after the scan is completed to enable side-by-side visualization of the same location on the retina for each visit. These comparison measurements from current and prior visits provide a thickness change map that assists the physician in determining the next steps for the patient. 6. It also must allow angle imaging and central corneal thickness measurements as well as ganglion cell analysis for glaucoma structural assessment. 7.Cube scan pattern: 512 x 128 with >67 million data points with 47um spacing between lines and 200 x 200 with >40 million data points with 30um spacing between lines
