- Scope of Work: Statement of Work Installation of GCX Mount for Axiomtek Workstation STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: Installation of GCX mounts for Axiomtek workstations. 2.Scope of Work: The contractor shall provide all resources necessary to accomplish the installation described in this statement of work (SOW). 3.Background: The ARK project in the OR and PACU requires the Axiomtek workstation to be mounted to both the Drager Anesthesia units and the walls of the PACU. (These mounts are currently used at Manhattan VA for the ARK system). 4.Performance Period: The contractor shall complete the work required under this SOW in 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer. If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. 5.Type of Contract: Firm Fixed Price STATEMENT OF WORK - the contractor shall provide labor, equipment and supervision to perform the following: 1.Install GCX Mounting hardware on 6 Drager Apollo Anesthesia units, 1Drager Narkomed GS unit, 1Drager Narkomed 6400, and 6 Wall mounts. 2. Contractor will provide all necessary equipment for the installation. GCX mounting hardware will be provided to the contractor with all necessary parts. During the period of the project, the contractor will have access to the site only during business hours of Monday to Friday, 8:00 AM to 4:30 PM. The contractor will need to coordinate with the government Contracting Officer/VA POC to arrange access to the VA Medical Facility. The contractor shall make every effort to cause minimum damage to any areas not included in this scope of work. Any damage caused will be repaired at the contractor's expense. Completion and Final Inspection -When project is complete a final inspection must occur prior to final payment to the contractor. Contractor must provide documentation to the Biomedical Engineering and Radiation Oncology Departments certifying the equipment and software set-up complete, appropriate parties trained and all modalities in full working order. Synopsis: Please note that this is NOT a request for proposals, the Government is seeking information for market research purposes only. The Department of Veterans Affairs, New York Harbor Health Care System is currently conducting a market survey for qualified firms capable of providing "Installation of GCX Mounting hardware on 6 Drager Apollo Anesthesia units, 1Drager Narkomed GS unit, 1Drager Narkomed 6400, and 6 Wall mounts, located throughout the Brooklyn-VA Medical Center." The North American Industry Classification Systems (NAICS) is 541512. Responses to this notice must be submitting in writing (email or fax) and must be received not later than March 31, 2015 at 12:00PM. No telephone inquiries will be accepted. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification//socioeconomic status (service-disabled veteran owned small business, veteran owned small business, HUB Zone, 8(a), small business or large business), relative to NAICS 541512. Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company/individual name, a service capability statement, examples of similar facilities which you have provided services to, DUNS number, address, point of contact and social-economic category (ex: SBVOSB, VOSB, 8(a), HUBZone, WOSB, EDWOSB, Small Business). If your firm is a Service-Disabled Veteran Owned or Veteran Owned Small Business, you must be CERTIFIED in VetBiz (see internet site: http://vip.vetbiz.gov). Contractor must be registered in Central Contractor Registry (CCR) (see internet site: http://www.ccr.gov). Responses to this notice must be submitted in writing (email or fax) and received not later than March 31, 2015 at 12:00PM. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement.
