Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only synopsis/solicitation; Quotes are being requested and a written solicitation will not be issued. This solicitation # W91YU0-15-T-0160 is issued as a Request for Quotes (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 and DPN 20150226. The associated North American Industrial Classification System (NAICS) code for this procurement is 325120. The Size Standard for small business is 1000 employees. This RFQ is being issued as Unrestricted. The dry ice is for use at Madigan Army Medical Center, Fort Lewis, WA. The estimated performance period is 1 May 2015 - 30 April 2016, with four 1 year option periods. PRODUCT DESCRIPTION: The vendor shall provide the following: CLIN 0001:Scheduled Delivery of Dry Ice. The vendor shall deliver 150lbs of nugget/pellet dry ice on Monday and Wednesday of each week, for a total of 104 deliveries in the base year. If delivery falls on a Federal Holiday, delivery shall be the following weekday. Pricing must include all fees associated with delivery. The period of performance is 1 May 2015 to 30 April 2016. Quantity Price Per Delivery Total Base Year Price 104 CLIN 0002:Unscheduled Delivery of Dry Ice. The vendor shall deliver 150lbs of nugget/pellet dry ice within 24 hours of the Government's request. Pricing must include all fees associated with delivery. The period of performance is 1 May 2015 to 30 April 2016. Quantity Price Per Delivery Total Option Year 4 Price 6 SALIENT CHARACTERISTICS 1.Each delivery of dry ice must be 150 pounds of pellets/nuggets. 2.Pellet/nugget size shall not exceed 2 inches in length. 3.If delivery falls on a Federal Holiday, shipment must occur on the following weekday. 4.The Government will provide a storage space and storage containers for the dry ice (two insulated containers available, each capable of holding 150lbs of nugget/pellet dry ice). The storage containers owned by the Government shall not leave the facility. It is acceptable if the vendor wants to provide a storage container to swap out.
