Description
This is a COMBINED SOLICITATION/SYNOPSIS for commercial services prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This solicitation has been prepared with the intent to award a Firm Fixed Price (FFP) contract, utilizing procedures under FAR part 13, for Commercial Items as described in the schedule below. This requirement is set aside 100% for small business concerns. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The Request for Quotation (RFQ) number N00259-15-T-0136 is issued as a Request For Quotation (RFQ). This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-80. QTY: One (1), Unit of Issue: Year (4 Quarters) As per the Statement of Work Unit Price: $___________ Total CLIN Price: $___________ Statement of Work Leagaioalii Mapu Medical Administrative Officer Naval Branch Health Clinic Kearny Mesa 8808 Balboa Avenue San Diego, CA 92123 619-645-0144 DELIVERY SERVICE OF ACCESS CONTROL SYSTEM GENERAL REQUIREMENTS The effort required hereunder shall be performed in accordance with this Statement of Work and in accordance with all other terms and conditions set forth herein. Contractor shall replace existing access control system with a 4-door controller IP link system that includes three (3) new card readers, all necessary programing, bringing the system on line, and a system test. The card system will be accessible by web portal to: program, add, and delete the HID access control cards, and to run reports. The location(s) of the card reader will be: (1) Front Entry with an alternate push button remote for front desk staff to accommodate patient and staff entry; (2) Staff Entry; (3) Rear Entry. Contractor will provide door controller with IP link, push button remote, and three new card readers. Contractor will provide HID access control cards. Contractor will provide training on the secure web portal to program HID access control cards, add cards, delete cards and run reports. Contractor will be responsible for all labor, materials, tools and transportation equipment necessary to install the access control system. Contractor will haul away all debris associated with the installation. Contractor will provide twelve months of hosting fees for the web portal access (if applicable to your product), and pricing for an additional two option years of hosting to be exercised at the governments discretion (if applicable to your product). FACTOR 3: Price; the government shall conduct a price evaluation of all technically acceptable offers with satisfactory past performance. Technical capability and past performance, when combined, are significant evaluation factors that will be most advantageous to the Government.
