Statement of Work Asbestos Abatement Project #1427 1.0INTRODUCTION/OVERVIEW The project involves the removal and disposal of floor tile and adhesive in the Mental Health Ward examination rooms, ENRM VA Medical center which have been determined to contain traces of asbestos. 1.1BACKGROUND In the course of an ongoing Veterans Construction Team (VCT) renovation of the Mental Health Ward, which includes approximately four thousand four hundred fifty square feet of office/examination rooms, an independent survey verified the presence of asbestos in the vinyl tile and tile adhesive in these rooms. (A copy of the report of this survey is available). The vinyl asbestos tile is bonded directly to a concrete substrate with asbestos containing adhesive and is covered, in all cases, with direct-adhered commercial carpet. 1.2 SCOPE The scope of the work is as follows: Survey the site conditions prior to the scheduled start date. Remove the existing commercial carpet in all office/examination rooms and dispose. Remove all vinyl asbestos tile in the office and examination rooms and dispose. Remove all asbestos containing vinyl tile adhesive and dispose. Conduct all work in compliance with applicable federal, state, and facility statutes and regulations. Provide on-site air quality monitoring throughout the entire removal process. This monitoring service shall be in addition to an independent air quality monitoring service which shall be provided by the facility at no cost to the contractor. Conduct all work after hours on a predetermined schedule which shall be submitted by the veterans Construction Team and approved by facility engineering service. Coordinate all abatement activities with Veterans Construction Team and the facility engineering service. 1.3 OBJECTIVES Remove all asbestos containing materials in the areas described in the scope of work and drawings. Contractor shall ensure that no one is exposed to asbestos fibers in excess of the levels defined by federal law. Contractor shall ensure no asbestos contamination occurs outside of containment established by the contractor. Contractor shall ensure all asbestos containing waste is disposed of in accordance with applicable laws. Contractor shall document all activities pertaining to this project in accordance with all applicable laws. 2.0REQUIREMENTS 2.1 TASKS Over a period of five consecutive weekends, beginning on Friday evening, the contractor shall establish an area of containment as defined by the drawing provided by VCT. These five separate containment areas correspond to the five phases of the abatement project. (Please refer to drawing D-2 "ABATEMENT PHASING", dated February 12, 2015). The containment shall comply with applicable regulations and shall encompass from four to eight offices and the adjacent common area needed to complete the work. The contractor shall establish an air quality monitoring station outside the perimeter and provide a qualified air quality monitor for the duration of each phase of work. The carpet and all asbestos containing materials shall be properly removed and disposed. This work must begin no earlier than 4:00 PM Friday evening and shall be completed no later than 12:00 PM Saturday afternoon. At this time the contractor shall verify "clean air' in the workspace and allow access to Veterans Construction Team workers. Adherence to this schedule is important so that sufficient time is available to replace the floor finishes and furniture to the affected rooms before 7:00 AM Monday morning. The contractor shall not be responsible for the removal or replacement of room furnishings. All documentation, including daily logs, shall be provided by the contractor to the Construction Team at the end of the final phase of work, but such documentation must be available for review by engineering service throughout the project. The series of tasks listed above shall be repeated on consecutive weekends until the project is completed. All debris must be removed from the jobsite and from the facility before the next phase of work begins. 2.2 DELIVERABLES The contractor shall provide the names, job titles, and documentation of the qualifications of all workers and the air quality monitor prior to the start of the project. The contractor shall deliver the services outlined within this statement, beginning not later than two weeks from date of award of contract, and shall continue on consecutive weekends until completed. An authorized representative of facility engineering service shall verify that the services are accepted. The contractor shall deliver, on completion of the project and before invoice submission, all documentation required by law. This documentation shall be provided to an authorized representative of the Veterans Construction Team, and will be identified to the contractor on the date of contract award. 2.3 SCHEDULE The period of performance shall begin within two weeks of contract award (on a Friday) and shall continue, uninterrupted (without just cause), until completion of all phases of work. 2.4 ASSUMPTIONS The contractor has an established and verifiable record of at least ten (10) successfully completed projects similar to the project outlined. The contractor holds all licenses, insurance, and certifications required by law for such services. The contractor has all resources readily available to provide the outlined services within the scheduled time. The Veterans Construction Team shall provide all resources necessary to remove and reinstall all office furnishings in accordance with the established work schedule. 4.0 SECURITY REQUIREMENTS The contractor shall notify the Veterans Construction Team prior to arrival at the facility. Contractor shall provide worker names and titles prior to the start of the project, including the supervisor who shall be the contractor's site representative, and shall ensure compliance with facility regulations.
