- SYNOPSIS: The Tennessee Army National Guard located at Volunteer Training Site-Smyrna 8th Ave Bldg 250 Smyrna, TN 37167 intends to issue a Request For Proposal (RFP) on or about April 14, 2015 to award a firm fixed-price contract for services, non-personal for Case Management Non-clinical and non-dental services. TN ARNG Surgeon's Office requires both case managers and administrative care coordinator support services. The case management position may be filled at a minimum by either a Registered Nurse (RN), Licensed Vocational/Practical Nurse (LVN/LPN) or Social Worker. The Contractor shall furnish all labor and services required to provide a wide range of Medical and Dental Non-clinical Case Management and related Medical Administrative support services The North American Industry Classification System (NAICS) code for this work is 561330. The small business size standard is $27,500,000.00. This requirement will be solicited as TOTAL SET ASIDE for SERVICE DISABLED VETERAN OWNED SMALL BUSINESS (SDVOSB). There will be no site visit or pre-proposal conference. Interested offerors must be registered in System for Award Managment (SAM) to be considered for award. The award will be based off of Lowest Price Technically Acceptable (LPTA) with factors of Technical Capabilities, Past Performance and Price.
