- Financial Data Past Performance Experience Questionarie Schedule of Items This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Department of Agriculture (USDA) Forest Service, National Forests in Mississippi, is soliciting proposals for Lake Liming and Fertilization of Turkey Fork Lake, Chickasawhay Ranger District, De Soto National Forest. This is a performance based services acquisition, indefinite delivery, indefinite quantity contract. The contractor shall provide all the labor, equipment, supplies, supervision, transportation, supplies (except those designated as contractor furnished) licenses, permits and incidentals necessary to perform liming and fertilization and continued water quality maintenance, 250 surface acres of Turkey Fork Lake, Chickasawhay Ranger District, De Soto National Forest in accordance with specifications and provisions of this solicitation. Work consists of the Liming and Fertilization to maintain water quality. The Solicitation Number is AG-447U-S-15-0027 and issued as a request for proposal that will result in a firm fix priced option year contract with a base period from the date of award through September 30, 2015 and two (2) one year option periods. A list of contract line items and option years is attached. Award will be made based on the best value to the government and can be made on initial offers. The following factors shall be used to evaluate offers: (1) Past Performance (2) Technical Approach with sub-factors (a) Method (b) Equipment (c) Quality Control Plan (d) Resource Protection ( f) Safety Plan and (3) Experience. This solicitation is set-aside 100% for small businesses. Service Contract Act wage rates apply. NAICS code for this requirement is.115310. All offerors who submit a proposal for this project must be registered in the Systems for Award Management (SAM) database and must have completed Online Representations and Certifications (ORCA) in accordance with FAR 52.204-8. Offerors who fail to register in SAM and/or complete ORCA will not be considered for award. Registration can be done via the internet at http://www.sam.gov. This solicitation incorporates those provisions and clauses that in effect through Federal Acquisition Circular 78 date January 8, 2015. The following provisions applies and included in the solicitation; FAR 52.212-1, Instructions to Offerors-Commercial, FAR 52.212-2 Evaluation - Commercial Items,, AGAR Clause 452.209-70 Representation by Corporation Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction - Alternate I, and FAR 52.212-3 Offerors Representations and Certifications- Commercial Items. The following clauses applies and are included in this solicitation: 52.212-4, Contract Terms and Conditions-Commercial Items, FAR 52.216-1, FAR 52.216-18, FAR 52.216-19, FAR 52.216-22, AGAR 452.216-73, FAR 52.217-9, AGAR 452.228-71, AGAR 452.237-74, FAR 52.236-6, FAR 52.245-1 and AGAR 452.236-73. Your offer must include the following: Schedule of Items, Contractor Financial Information Sheet, Technical Proposal, Past Performance Questionnaire, Proof of Insurance, AGAR Clause 452.209-70 Representation by Corporation Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction - Alternate I, and FAR 52.212-3 Offerors Representations and Certifications- Commercial Items must be completed and included with offeror.
