Description
This is not a request for bids, quotes, or proposals. This is a Sources Sought notice only for the identification of potential vendors to provide all necessary labor, materials, equipment, and supervision necessary to produce and deliver Fresh Milk and Milk Products; i.e. such as but not limited to: the items listed below with a tentative delivery schedule of 3-5 days per week to the below VA Medical Centers and locations. Please list which Medical Centers your company can support (every medical center in not required as this is just to see capabilities of coverage). 502- Alexandria VAMC (2495 Shreveport Hwy Ste 71, Pineville, LA71360) 520 - Biloxi VAMC (400 Veterans Ave, Biloxi, MS 39531) 564 - Fayetteville VAMC (1100 N College Ave, Fayetteville, AR72703) 580 - Houston VAMC (2002 Holcombe Blvd, Houston, TX 77030) 586 - Jackson VAMC (1500 E Woodrow Wilson Ave, Jackson, MS 39216) 598 - Little Rock VAMC (4300 W 7th St, Little Rock, AR 72205) 598 - North Little Rock VAMC (2200 Fort Roots Dr, North Little Rock, AR 72114) 623 - Muskogee VAMC (1011 Honor Heights Dr., Muskogee, OK 74401) 629 - New Orleans VAMC (1601 Perdido St., New Orleans, LA 70112) 635 - Oklahoma City VAMC (921 NE 13th St., Oklahoma City, OK 73104) 667 - Shreveport VAMC (510 E Stoner Ave, Shreveport, LA 71101) The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification (HUBZone, 8(a), Small, Small Disadvantaged, Small Veteran Owned, or Service Disabled Veteran Owned Small Business) relative to NAICS 311991 (size standard of 500 employees or less). Do have these products on a GSA/Federal Supply Schedule? Please list business size. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. All information submitted in response to this Sources Sought notice become property of the Government. Interested and capable respondents are requested to provide the information below. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number, list of available products, the geographical location the respondent is able to service and a list of the respondent's establishment's certifications from private, State and or local agencies documenting the safety and sanitary conditions and procedures. Responses to this notice should be emailed to Billy Morris, Contracting Officer at e-mail address: billy.morris@va.gov. Telephone responses will not be accepted. Please DO NOT respond in the 'interested vendor' tab as the Contracting Officer does not have access and responses there will not be seen. Responses must be received in writing no later than 4:00PM CST, 16 April 2015. This notice is to assist the VA in determining sources only. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov.
