Description

The North Florida I South Georgia Veterans Health System (NF/SG VHS) is a comprehensive and integrated health care delivery system that provides quality clinical programs and services to veterans. We offer primary health services as well as highly specialized, technologically advanced clinical care. NF/SG VHS consists of two hospitals, three large satellite outpatient clinics, and several community based outpatient clinics. The NF /SG VHS requires the services of an outside laboratory to perform specialty tests it is unable to perform in-house to assist in this mission. This reference laboratory must include the ability to perform flow cytometry, cytogenetics, fluorescence in situ hybridization (FISH), & molecular testing. The anticipated annual volume of these tests are 400, 300, 400 and 150, respectively. It is anticipated that this procurement will be 100% set-aside for small businesses and involve a base year and two option years. Additionally, offerors will be required to provide documentation to assist in the evaluation of their proposal including: 1. Quote for each type of testing in the number specified in this RFQ. Quotes must be inclusive of all shipping, handling or other direct or ancillary charges. 2. Key Personnel: Resume(s) not to exceed two pages for each member of the vendor's organization who will play a principal role in the execution of the work under this contract/order. 3. Licensing: Copies of all federal, state or local licenses required to perform the services called for under this contract/order. 4. Sub-Contracting Plan (if sub-contractors will be used). Also include items in 2 and 3 above for sub-contractor personnel. 5. Capability Statement: Specifically addressing the vendor's ability to perform Flow Cytometry, Cytogentic, Fluoresence in Situ Hybridization (FISH) and Molecular testing. Include experience in each testing application. Provide estimated turn-around time for each and note any provisions for expedited service and the manner in which results are relayed to the VA. 6. Statement of Compliance with Privacy Laws: Provide a statement affirming the vendor has no previous (within 15 years) violations of HIPPA, Privacy Act or other regulation pertaining to the protection of Patient Information or Personally Identifiable Information (PII). If an incident(s) has occurred, provide a summary for each of the information divulged, extent of the compromise, cause (accidental or intentional) and action taken to preclude a reoccurrence. The Request for Quotes is anticipated to be published o/a 7 Apr 2015 and remain open for offers until o/a 19 Apr 2015.
