- The purpose of amendment 0002 is to address questions submitted in response to Solicitation VA247-15-R-0355, Project 509-11-106, A/E Design Renovate and Upgrade Operating Rooms, Phase 1. Amendment 0002 A/E Design Renovate and Upgrade Operating Rooms, Phase 1 2 April 2015 Question 1: See Amendment 0001 Question 2: There is a request for Commissioning of the project, do I need to include the company that will be performing the commissioning as part of my submission or is that something I provide later and acknowledge the request in the submission? Answer : The submission that is made should address all selection criteria factors within the submitted SF330 packages. Question 3: Do I need to provide a medical equipment planner as part of my submission, with their specific information, or is that being provided through the VA? Answer: The medical equipment planner is not being provided by the VA. Prospective firms shall address all selection criteria factors within their submitted SF330 packages. Question 4: What is the submission date for the SF330 packages. Answer: Interested firms must submit four (4) copies and 1 CD of their SF 330 no later than 20 April 2015 at 1:00pm EST to Theresa Elkins, Contract Specialist, Veterans Affairs, 501 Greene Street, Suite 2, Augusta, GA 30901 Question 5: Are we allowed to submit previous Past Performance Questionnaires as many times Project Managers or Contracting Officers most familiar with a project may have moved to a different facility, agency or have retired. Answer: Previous Past Performance Questionnaires cannot be submitted. Past Performance Questionnaires must have a current date and be written by a person familiar with the project even if he or she no longer works at that particular facility or is retired.
