This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a solicitation W91YU0-15-T-0171 of Purchase Request 0010665101 is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-80 The associated North American Industrial Classification System (NAICS) code for this procurement is 339115, Ophthalmic Goods Manufacturing; Size Standard for small business is 500 employees. This RFQ is being issued as Firm Fixed Price, UNRESTRICTED. All responsible sources may submit a quotation which shall be considered by the agency. PRODUCT DESCRIPTION: All responsible Contractors shall provide an offer of BRAND NAME OR EQUAL for the Zeiss Cirrus Photo 600 model and Cirrus Photo FAF License. LINE ITEM: 001 Nomenclature: Zeiss Cirrus Photo 600 model UI: EA QTY: 1 BRAND NAME OR EQUAL LINE ITEM: 002 Nomenclature: Cirrus Photo FAF License UI: EA QTY: 1 BRAND NAME OR EQUAL SALIENT CHARACTERISTICS Optical Coherence Tomographer (OCT)/Anterior Segment Photo/Posterior Segment Photo 1. The OCT must have the ability to provide high definition OCT scans of the retina, including the macula, optic nerve, and retinal nerve fiber layer. Fundus image and data density must provide clinically relevant views of the retina. 2. The device needs to be easy to use and does not require a highly-skilled technician. 3. The unit must be able to scan customized areas of the retina other than the posterior pole. 4. The OCT needs to be able to capture entire scan cube no less than 2.4 seconds. 5. The retinal imaging and OCT must have an integrated design (OCT/camera in one). Requires a small footprint to fit in the small available space of the clinic. 6. The device needs precise registration of OCT scan to fundus image using advanced algorithms. 7. The OCT is required to have auto centration of RNFL analysis. 8. Have the capability to process high data density, minimum of 1024 data points captured over a 2mm depth to create a tissue density profile (A-scan), 512 A scans creating a cross sectional image (B-scan). 128 B scans captured to create a cube with over 67 million data points. 9. Be able to capture data from single macula cube scan providing high resolution cross hair images; 3D layer map of inner limiting membrane (ILM) and retinal pigment epithelium (RPE); 3D retinal thickness map; 3D cube imaging; OCT fundus image with overlay of thickness map, OCT fundus. 10. Provide glaucoma scan of retinal nerve fiber layer (RNFL) thickness map and deviation map with comparison to normative data; TSNIT (Temporal, Superior, Nasal, Inferior, Temporal) graph and both eye analysis. Adjustable five-liner raster scan providing exquisite High Definition B-scan image. 11. Be able to capture retinal images through a no less than 3.3 mm pupil size. This eliminates dilation of patients. 12. Have advanced positioning and focusing aid, as well as auto-focus. This reduces user-training time. Auto focus enables a smooth and fast workflow while reducing operating steps resulting in convenience and greater efficiency. 13. Have auto flash for retinal imaging. Must be able to acquire images with the correct exposure nearly 100% the first time providing the opportunity to acquire more images non-mydriatically (without dilation). 14. Include a built-in medical scientific sensor which is calibrated for retinal images. 15. Must have an internal fixation target with various fixation points and free-hand mode. 16. Must have telecentric optics to neutralize the effects of patients's emmetropia, eliminating the potential for image distortion and delivering repeatable and precise measurements of retinal physiology resulting in true pathological size regardless of focus. 17. Must have stereo capture mode for fundus stereo photography. 18. Must have advanced imaging options for fundus autofluorescence and ICG angiography. This encompasses a combination of a non-mydriatic fundus camera with superior image quality and an easy-to-use camera for fluorescein angiography. This makes it practical for the detailed diagnosis and management of typical diseases of the eye, such as diabetic retinopathy, glaucoma and age-related macular degeneration, for easy and fast monitoring of AMD patients, and for clinicians who want to perform ICGA procedures. 19.
