Description
This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation Announcement, request for proposal or request for quote. This does not obligate the Government awarding a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested capable of performing the work. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. STATEMENT OF WORK 1.Introduction and Overview Title of Project: 526-15-1-636-0122 - Replacement of Curved Monolithic Glass The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as otherwise be specified for the above project. 1.1Background: The Department of Veteran Affairs at James J. Peters VA Medical Center 130 W. Kingsbridge, NY 10468 is seeking services to repair and replace 3 different sections of curved monolithic tempered glass that is broken. The Besam Entre Matic RDB-2 revolving door is located at James J. Peters Medical Center Building 100. The door is 20 years old and the glass needs to be replaced. 1.2Scope of Work: Product Installation - The BESAM/ASSA ABLOY RDB (UniTurn) revolving door system is a 2 wing-automatic revolving door system with a nominal diameter of 14 feet. 3 x ¼" clear safety tempered glass windows approximately 4ft x 7ft curved need replacement. Vendor shall provide all goods and services to install and replace 3 x ¼ inch tempered glass windows. 1.3Objectives: The BESAM/ASSA ABLOY RDB (UniTurn) revolving door replacement of tempered glass. "Complete removal and replacement of 1x curved monolithic tempered safety glass ¼ inch thick with measurements approximately 4 feet long x 7 feet high. The exact dimensions shall be verified by installing contractor. "Complete removal and replacement of 1x curved monolithic tempered safety glass ¼ inch thick with measurements approximately 4 feet long x 7 feet high. The exact dimensions shall be verified by installing contractor. "Complete removal and replacement of 1x curved monolithic tempered safety glass ¼ inch thick with measurements approximately 3 feet long x 7 feet high. The exact dimensions shall be verified by installing contractor. "Vendor shall provide all tools, labor, and installation services to replace broken glass. "Vendor shall assume all responsibilities installing tempered safety glass. "Vendor shall assume all responsibilities for proper door functions including safety sensors, wiring and faulty battery packs, rotation speeds, responses to revolution encoders, and fire thermal overloads. "Functionality of all door mechanics must be tested, and approved by COR after installation. "All damaged glass shall be removed and carted away from the facility. "No tools, equipment, or objects shall restrict movement through secondary front entrances while installation is in progress. 1.4Performance Period: [Firm-Fixed-Price] The contractor shall complete work required under this SOW in 14__ calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). 2.References: The RDB-2 Revolving Door Troubleshooting manual shall be provided upon request. This document does not explain installation procedures, but does depict scope and size of the door which has already been stated in the SOW. 3.Tasks: The vendor shall perform installation by all efficient means. Normal work hours apply and delays to service should be reported to building maintenance staff and security. The scheduled installation will be coordinated by the Vendor and the COR (requesting office) so that the VA Police are notified within 48 hours of the scheduled installation. 4.Progress/Delays: Any delays will be reported immediately to the COR. All incidents outside the Scope of Work shall be reported to the COR. No tools or equipment shall restrict movement of patients, VA staff, or visitors through secondary doorways in the front entrance while installation occurs. 5.Transmittal/Delivery/Accessibility: The contractor shall provide [1] hard copies of each deliverable and one electronic version. 6.Place of Performance: The products and services will be delivered and performed at James J. Peters VA Medical Center, 130W. Kingbridge Rd. Bronx, NY 10468. Important Information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. No sub-contracting opportunity is anticipated. Notice to Potential Offerors: All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM located on the web at www.sam.gov). It is desirable that any Offeror to have completed their business Online Representations and Certifications Application (ORCA). Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the Department of Veterans Affairs Center for Veterans Enterprise VetBiz Registry located at http://vip.vetbiz.gov. All interested Offerors should submit information by mail or ground carrier to: Northport VA Medical Center, Network Contracting Office, (632/NCO), Bldg. 10 Room 216E Northport, NY 11768 or by email to john.redmond2@va.gov. All information submissions to be marked Attn: John Redmond, Contracting Specialist. Responses are due BY 9:00AM. Eastern Standard Time (EST) Wednesday April 8, 2015. After review of the responses to this announcement, the Government intends to proceed with the acquisition and a formal synopsis and subsequent solicitation announcement will be published. The Governments intent is to solicit a Firm Fixed Price, Best Value Procurement.
