- This file includes the Solicitation, clauses, and Scope of Work This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912P6-15-T-0003 and is issued as a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-23. The Government reserves the right to make an award on the initial quote without discussions on this procurement. Award will be made to the responsive and responsible offeror who submits the lowest price to perform the services set-forth in the Bid Schedule. The NAICS code for this requirement is 212312, with a small business size standard of 500 employees. This acquisition is unrestricted to business size. The provisions at 52.212.1, Instructions to Offerors-Commercial applies to this acquisition. Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with this offer. The clause 52.212-4, Contract terms and conditions-Commercial Items applies to this acquisition. The clause 52.212-5, Contract Terms and Conditions required to implement Statues or Executive Orders-Commercial Items applies to this acquisition. The following clauses listed in 52.212-5(b) and (c) are included by reference: 5.222-21; 52.222-22; 52.222-23; 52.222-41; 52.225.3; and 52.232-34; 52.232-16. This acquisition also incorporates the Clause included in the Scope of Work/Statement of Work. SCOPE OF SERVICES: The contractor will be required to furnish and deliver approximately 424 tons of RR 4 and 212 tons of CA 5 meeting or exceeding Illinois Department of Transportation (IDOT) aggregate material specification criteria. The material must be delivered to the locations using delivery vehicles. The material should be quarried; the use of recycled material is not permitted. PROJECT SITE VISIT: All offerors are encouraged to visit the project site to familiarize themselves with delivery vehicle entry and exit locations and the types of delivery vehicles required to be furnished to satisfy the delivery requirements of the contract. Project site visits can be arranged by contacting Adam Tennant at 312-846-5593 during normal business hours. DELIVERY POINT: The delivery point is the Chicago District Stone Storage Dock, on the north side of the mouth of the Calumet River, in Chicago, IL. The nearest street address is: 8555 South Green Bay Avenue, Chicago, IL. This is the location of the public street intersection at East 86th Street, and South Green Bay Avenue in the City of Chicago. It is at this location where a guarded gate is located. However, the access gate for equipment is actually located at the 89th Street gate of this facility. A USACE representative will be needed to escort the driver into the facility. The contractor is advised that on weekends, the on-duty guard may close this gate during periodic site patrols. RESPONSE DOCUMENTS, DUE DATE AND TIME: a. The Contractor SHALL PROVIDE ITS PRICING for ALL line items included in the Bid Schedule of this Solicitation (See Section B). -FOR ALL WORK TO BE ACCOMPLISHED UNDER A BID ITEM, THE CONTRACTOR SHALL ENSURE THAT ITS LINE ITEM PRICING IS ALL INCLUSIVE. b. The Contractor SHALL complete Section K of this Solicitation and submit the completed Section with its price quote. c. Responses to this Solicitation shall be submitted by Friday, 10 April 2015 no later than 4:30 p.m. CST. d.
