Description
Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80. The NAICS code is 541350, with a size standard of $7.5 million. This is being conducted as a Veteran Owned Small Business Set aside. This requirement is being solicited in accordance with FAR Parts 12 and under the Authority of FAR Subpart 13.5. The vendor shall provide a National Board Commissioned inspector to perform annual internal and external inspections of low pressure fire tube boilers and Annual and Semi Annual Safety Device Testing per the VA Boiler Plant Safety Device Testing Manual (3rd Edition) on Fourteen (14) Low Pressure Fire Tube Boilers located in Buildings 211, 212, 214 and 216 located at the VA IIliana Healthcare System at 1900 East Main Street, Danville, II 61832. The VA Boiler Plant Safety Device Testing manual shall be provided as an attachment to the solicitation. Services that shall be provided: 1. Vendor shall provide labor, equipment and materials to complete all tests required by the VA Boiler Plant Safety Device Testing Manual (3rd edition) for 14 low pressure fire tube boiler's and associated plant safety devices. 2. The following shall the required semi-annual operational tests required for Boiler Safety devices: a. High steam pressure cut out (recycle) b. High steam pressure cut out (non-recycle) c. Steam safety valves (raise boiler pressure until valve pops) d. Check gas vent for leaks e. High gas fuel pressure shut off f. Low gas fuel pressure cutoff g. Gas fuel safety shut off valve proof of closure h. Leak test gas fuel safety shut off valves i. High fuel oil temperature cut off (heated Fuel) J. Low fuel oil temperature cut off (heated fuel) k. Low atomizing pressure for fuel oil m. High fuel oil pressure cut off n. Low fuel oil pressure cut off o. fuel oil safety shut off valves proof of closure p. Leak test fuel oil safety valves q. Low pilot gas pressure cut out r. Forced draft fan motor interlock s. Boiler outlet damper wide open for purge t. Purge airflow interlock u. Timing for pre purge v. Timing for post purge w. Igniter timing x. Low fire position interlock y. Combustion air interlock z. Main flame out; I. e., time to close valves aa. Ignition flame out; i.e., it is time to close valves bb. Minimum igniter flame test cc. scanner not sensing ignition spark dd. Low oxygen alarm and/or cut out ee. Pre purge setting of flue gas recirculation damper ff. Interlock of outside air damper with burner control gg. Adjust burner combustion settings and calibrate oxygen trim hh. Check vibration of burner fans ii. Calibrate instrumentation, monitoring and control systems jj. Check combustion gas leaks into boiler room kk. Check furnace pressure II. Check operation of gas pilot 3. The following shall be the required annual inspections: a. Check for scaling of tubes and inside of boiler b. Check for Oxygen pitting of tubes and boiler c. Check inside of tubes and tube sheets for leakage d. Check inside of tubes for excess carbon e. Check refractory for excessive cracking and missing sections f. Inspect interior and exterior, supports, closures, accessories, valves and controls g. Calibrate pressure gages and thermometers h. Boiler fouling and combustion gas flow check i. Tube leak check j. Steam safety valves (accumulation test at high fire) k. Clean waterside of boilers 4. The following shall be semiannual requirements: a. Conduct review of General Plant Safety and Reliability b. Check water treatment in all buildings c. Check Carbon Monoxide alarms in all buildings d. Check combustible gas alarms in all buildings 5. Vendor shall coordinate with Don DeMoss, VA Engineering (217) 554-5994 to schedule work prior to arriving at site. Work shall be done in such as manner as to allow continuation of service to all campus buildings. Time period for Inspections Work shall be scheduled semiannually in March and September with 50% of the boilers undergoing annual inspection during each time frame allowing continuation of service to the facility. NOTE: Offeror shall hold the prices in its quote firm for 90 days from the date specified for receipt of quotes. Quote Submission Information and Evaluation Criteria Offerors shall include the following with their offers in order to be considered for award: 1. Offers are required to completed Blocks 17, and 30(a-c) of the SF1449; Contract Administration Data; the Price Schedule; Quote Submission Information; and Offeror Representations and Certifications - Commercial Items. In doing so, the offeror accedes to the contract terms and conditions as written in the RFQ. Offeror shall submit acknowledgement of all solicitation Amendments issued and the information identified in paragraph 4 below. 2. Quote Format and Structure - To provide a basis for sound evaluation by the Government, offerors must submit a complete quote. The information provided shall be concise, factual, complete, and demonstrate the offeror's capacity and capability to satisfactorily perform the requirements outlined in the solicitation. Offers will be considered only from offerors that are regularly established in the business and in the judgment of the Government, are deemed financially responsible and able to show evidence of appropriate experience. 3. Quote Submission - Offerors shall submit one electronic quote or furnish one CD copy of the quote. The SF 1449 shall contain a "wet" signature. 4. Items to be evaluated are: a. Technical: i. The Offeror shall demonstrate understanding of the requirements of the Statement of Work, and produce an offer that will meet those objectives. The offer will be evaluated based on the qualifications, experience and education of the proposed providers in addition to the contractor's approach to meeting the needs of the Statement of Work. ii. Provide a list of the names Key Personnel to be utilized in the performance of this contract, along with a copy of their resumes/curriculum vitae, and a description of the particular services to be provided by such personnel. iii. Relevant experience regarding the requirements of the Statement of Work iv. Evidence of National Board Commissioned inspector Certification. v. Offerors shall indicate which subcontractor, if any, and provide the necessary information for the subcontractor's personnel. b. PAST PERFORMANCE: Past performance questionnaires received from references - minimum of 3 and a maximum of 5 - for similar contract service. Offeror is responsible for ensuring references submit the completed past performance questionnaire directly to the Contracting Officer at the issuing office for receipt prior to the closing date and time for receipt of the quotes. Questionnaire, listed in Section D, is attached to this the solicitation. oPast performance evaluations will be conducted using information provided with the offer, information obtained from references, information obtained from the Federal Awardee Performance and Integrity Information System (FAPIIS) and/or the Past Performance Information Retrieval System (PPIRS), and information from any other sources deemed appropriate. oWhen evaluating past performance the government may consider the currency and relevancy of the information, the source of the information, the context of the data provided, and the general trends in the Offeror's performance. The government may take into account past performance information regarding predecessor companies, key personnel who have relevant experience and/or subOfferors that will perform major or critical aspects of the requirement. Offerors can provide information on problems encountered on the identified contracts and the offeror's corrective actions. o An offeror that has no available relevant past performance history will not be evaluated favorably or unfavorably. Offerors are cautioned that the Government may review available past performance data available in the PPIRS. The Government reserves the right to obtain past performance information from any available source and may contact customers other than those identified by the Offeror when evaluating past performance. o Since the Government may not necessarily interview all of the sources provided by the Offerors, it is incumbent upon the Offerors to explain the relevance of the data provided. Offerors are reminded that the burden of proving low performance risk rests with the Offerors. c. Price 5. Failure to submit the necessary information may render the offer unacceptable, in which case it will not be considered for award. Quotes will be evaluated in accordance with the criteria set forth above. 6. Basis for Contract Award: This is a best value award conducted in accordance with Federal Acquisition Regulation (FAR) 13.106-2(b) and under the authority of FAR Subpart 13.5. The government will select the best overall offer, based upon price and other factors. Contract award will be made to the offeror whose offer is determined to represent the best value to the government. 7. Submission of Proposals: The quote shall be submitted via U.S.P.S regular mail, commercial overnight delivery or hand-delivered. It may also be emailed to Kevin Adkins, Contracting Officer, at kevin.adkins@va.gov.The quote shall be addressed to the office specified in the solicitation and shall show the time specified for receipt, the solicitation number and name and address of the offeror.
