Description

STATEMENT OF WORK A.GENERAL INFORMATION 1.Title of Project: Provide Bulk Liquid Medical Grade Oxygen and Oxygen Tank rental. 2. Scope of Work: The contractor shall provide all labor, materials and supplies necessary to supply medical grade oxygen to the Veterans Affairs Medical Center. The contractor will provide rental of a 6000 gallon main tank, 900 gallon reserve. The contract shall include all required components of the bulk oxygen supply system to include fittings, values, pressure relief values, gassifiers and other components to provide a useable system that will connect to Government owned piping, preventive maintenance on tanks and manifold system to include testing switches and alarms annually, annual training on proper filling procedures to station personnel onsite, and inventory management of the bulk systems on how to monitor system. The supply systems will be located at the existing pad at the rear of the building 1670 Clairmont Rd. Decatur, Ga. 30033. The system must be maintained to ensure uninterrupted supply of oxygen to the medical center. During any tank change out process, the supply contractor will also provide medical oxygen temporary liquid bulk trailer that includes main and reserve tank connected to the emergency connection point at the boiler room loading dock. Contractor is responsible for all cost, materials, equipment, and Crain rental for the complete installation and removal of all tanks and equipment that have been installed. Contractor must accomplish the deliverables described in this statement of work (SOW). 3. Background: New contract. 4. Period of Performance: Base year: 6/1/2015 through 6/30/2016 with optional 4 years. 5. Type of Contract: Fixed Price B.DELIVERY LOCATION: Department of Veterans Affairs Atlanta VA Medical Center, 1670 Clairmont Rd. Decatur, Ga. 30033 ATTN: Tim Little C.SCHEDULE OF DELIVERABLES/INSTALLATION: Frequency of Fill: As Needed or Emergency delivery when main tanks reaches low limit of 100 inches or reserve reaches low limit of 50 inches. Provide all equipment required to fill tanks as needed. Hours of Delivery: Between 3:00 pm and 6:00 am seven days a week to include holidays. Instructions: Contact Mr. Little in boiler plant at (404)-321-6111 ext. xxxx prior to arrival for access. The site visit will be conducted at the Atlanta VA Medical Center located at 1670 Clairmont Road Decatur, GA 30033-6137. Contractors shall report to the ground floor Engineer Conference room GC125E for further instructions.
