Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is VA257-15-Q-0759. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72, effective January 30, 2014. The associated North American Industrial Classification System Code (NAICS) for this procurement is 339112 with a small business standard of 500 employees. This acquisition is solicited as a set-aside for small business. Network Contracting Office 17 requests responses from qualified sources capable of providing the following item(s): ITEM NUMBERDESCRIPTION OF SUPPLIES/SERVICESQUANTITYUNITUNIT PRICEAMOUNT 1XTRAC Ultra Plus or Brand Name or Equal, Purchase Warranty Coverage: The XTRACr Laser is warranted under the XTRAC Laser Warranty to be free from defects and workmanship under normal conditions and use. Details of the warranty coverage are in the XTRAC Laser Warranty, which comes with the laser. All service calls, hardware components and software are covered during the Warranty Period without limitation with the exception of the Laser Chamber which is limited by the table below: Chamber Warranty Ultra Laser (\R\treatments) Velocity Laser (\R\treatments) 75 M Pulses (\R\3,750) 112.5 M Pulses (\R\5,625) (Approximate number of treatments based on average of 20,000 pulses per treatment. Contractor will schedule a Field Service Engineer to remove the billing software once full payment is received.
