Description

PRE-SOLICITATION NOTICE Solicitation VA69D-15-B-0706 Project 676-14-112 111 Site Prep Cath Lab Clement J. Zablocki VAMC in Milwaukee, WI Scope of Work: The composition, design and site preparation for renovation for the new Cath Lab equipment in rooms 5503, 5503A, 5503B and 5547 with associated demolition, structural, architectural, mechanical, electrical, low voltage systems (Nurse Call, MATV, Telecommunications, Public Address, Security) design, medical gas relocations/upgrades, furniture layout, and equipment (Contractor furnished+Contractor installed (C+C), VA furnished+Contractor installed (V+C), and VA furnished+VA installed (V+V)). The design is based on the VA Cath Lab design guide and Cath Lab preferences. Rooms 5503, 5503A, 5503B and 5547: to receive demolition of equipment and a full interior remodel to accommodate new equipment to include ceiling, lights, walls, and floors. Room 5547 will be divided into two room, 5547A and 5547B. 5547A will be designated as Hand Washroom and will receive a hand scrub sink and associated equipment currently provided in 5503. Room 5547B will be designated as an Equipment Room to receive equipment required for the Cath Lab imaging system. A sliding door will be provided for ease of movement between Hand Washing Room 5547A and Treatment Room 5503. Room 5546: Room to be provided with new finishes, nurse call system at patient bedside and intercom communication with Treatment Room 5503. This project shall be completed within 365 calendar days of NTP. The NAICS code for this procurement is 236220 with a size standard of $36.5 million dollars. The project magnitude range is between $500,000 and $1,000,000. This Pre-Solicitation Notice is NOT a request for proposals or submittals of any kind. The VA plans to issue the official solicitation, VA69D-15-B-0706 on or about 23 April 2015. The solicitation will be posted to https://www.fbo.gov. Pursuant to Public Law 109-461, a Solicitation will be issued as a 100% Service Disabled Veteran Owned Small Business (SDVOSB) Set-aside. In order to be considered for award, the contractor shall be CVE-Verified as an SDVOSB vendor in https://www.vip.vetbiz.gov by the date and time specified for receipt of proposals. No other sources will be considered. All interested parties shall have a current registration and current annual On-line Representation and Certifications Application (ORCA) in the System Award Management (SAM) database https://www.sam.gov/prtal/public/SAM prior to award.
