Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. The solicitation number for this procurement is W912JD-15-Q-2006 issued as Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-80 and DFARS DPN 20150326. The North American Industry Classification System is 238290 and the business size standard is $15,000,000. This requirement is a total small-business set aside IAW FAR 19.502-2(a). SUBMISSION AND EVALUATION OF PROPOSALS: To assure timely and equitable evaluation of the proposal, offerors shall follow the instructions contained herein. Offers shall be submitted as a single electronic package to the Primary Point of Contract email address not later than the date and time identified within this notice. The offer must be complete, self-sufficient, and respond directly to the requirements of this solicitation. Offers shall be evaluated using a Best Value determination when considering price, technical capability and past performance IAW FAR 13.106-1(a)(2), FAR 13.106-2(b) and FAR 52.212-2. ________________________________________ BID SCHEDULE OF SUPPLIES: CLIN 0001 - Air Handler Insulation Non-Personal Services: Refurbish 15 Air Handler Units (AHU) by removing degraded interior insulation within the Air Handler Units located in the Penthouse with a three part sprayed in epoxy process IAW the Statement of Work dated, 9 March 2015. Shall conform to NFPA 90A/Fire Code Compliance and ASHRAE 62. Offerors' shall provide technical data proving that their proposed process conforms to the ASHRAE and NFPA standards. Offerors' shall provide a detailed summary of their application process with their offer. BRAND NAME OR EQUAL One or more items in this RFQ have been identified by a manufacturer's part number follow by the words quote mark Or Equal quote mark. Offerors are encouraged to quote other manufactures products that meet the minimum salient characteristics identified in the item descriptions. If you are submitting an equal item(s) as part of your quote, IAW FAR 52.206-11 -- Brand Name or Equal, you must identify equivalent item(s) when the quote is submitted and provide product technical information that demonstrates the item(s) meet the salient characteristics of the Bid Schedule of Supplies. Failure to comply with this requirement will result in your quote being considered non-responsive. PERIOD OF PERFORMANCE: 90 days - ARO; All work to be completed no later than 30 September 2015 or 90 days, whichever occurs first. F.o.b. - Destination. If Offeror is not able to meet this scheduled, the Offeror shall propose an alternative delivery schedule the Offeror can meet. Shipment shall be to the following address: Mark For DoDAAC - HQ0443 DFAS-Limestone Attn: Mr. Michael Kinney 27 Arkansas Street, Suite 104 Limestone, ME 04751-6216 SITE VISIT:A site visit will be conducted on 22 April 2015 at 12:00:00 PM EST, DFAS-Limestone, 27 Arkansas Street, Limestone, ME 04751. This is the only site visit that will be conducted. Interested parties shall provide the completed EAL template three days prior the site visit or risk being denied entry to the facility. IAW FAR 52.237-1, Site Visit, Offerors' or quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. QUESTIONS AND ANSWERS: All questions and answers must be in writing. WAGE DETERMINATION: Currently, Wage Determination [2005-2241] Revision No. [16] dated [12/22/2014] for [Aroostook County] is applicable. Rates can be found at http://www.wdol.gov/wdol/scafiles/std/05-2241.txt?v=16. However, the current prevailing Wage Determination will be incorporated at the time of award. In the event the current prevailing Wage Determination is changed by the Department of Labor after the closing date of this solicitation, the updated Wage Determination will be incorporated by amendment and issued to all Offerors that have not been eliminated from the competition. If applicable, these Offerors will be given a reasonable opportunity to provide revised proposals solely to amend proposals due to Wage Determination changes. This does not constitute and shall not constitute as discussions if award without discussions is contemplated. For the purposes of this offering, it is implied that, that any and all parties agree to a Criminal History Background Check once entered into any contract/task order. Any documents that are provided and found fraudulent to include, but not limited to driver's licenses, contracting licenses, etc., will result in an arrest and potential termination for default from the contract. All contractors are subject to search in order to obtain entry onto the installation and while working on the installation. Any refusal to submit to a search of their person or vehicle will be deemed as a violent act of aggression against the installation and will be cause for dismissal from the installation and revocation of future privileges and installation access. MULTIPLE AWARDS: The Government reserves the right to place awards to multiple venders. Multiple awards may result from this RFQ if no one vendor is able to meet the Government's delivery schedule. TECHNICAL INFORMATION: Offerors quoting an equivalent item shall include the Manufactures Technical Data with their offer. Technical data must clearly demonstrate the product offered meets the minimum specifications identified in the Bid Schedule of Supplies. Offers of equivalent products without the required technical data shall be considered non-responsive. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
