- ATTACHMENT A THIS IS A REQUEST FOR INFORMATION (RFI) ONLY. This is not a Request for Proposal (RFP) or Request for Quotation (RFQ). In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: an automated blood culture analyzer with continuous surveillance and monitoring capability to detect bacterial and fungal growth in blood culture specimens. (Refer to attachment A for additional information.) Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from April 10, 2015 through April 20, 2015. If interested please submit the following: 1.If your company can successfully perform the services 2.If your company is a Small or Large Business 3.Indicate if your company is registered in SAM 4.Your companies Data Universal Number (DUN) This RFI is for planning purposes only. This sources sought notice / (RFI) is not to be construed as a commitment by the Government, nor will the Government pay for any information solicited.
