Description
The National Library Services for the Blind and Physically Handicapped (NLS/BPH) within the Library of Congress (LOC) requires the following Braille magazines: Braille Chess Magazine-125 copies per quarter, giving a total of 500 copies. Short Stories-465 copies per month, giving a total of 5580 copies. Braille Music Magazine-220 copies per month, giving a total of 2640 copies. Braille Conundrum-475 copies per month, giving a total of 5700 copies. LOC intends to issue this requirement as a sole source purchase order the Royal National Institute for the Blind located in the United Kingdom. Any firm that wishes to be considered for this requirement must submit a capability statement that demonstrates its ability to meet all the requirements described in the attached draft SOW. The capability statement must not exceed five (5) double-spaced pages and shall include the following information: 1) Company Name, DUNS Number, Address, Point of Contact, phone number and email address; 2) Corporate structure (corporation, LLC, sole proprietorship, partnership, limited liability partnership, professional corporation, etc.); 3) References, including name, phone number and email address of individuals who can verify the demonstrated capabilities identified in the responses; 4) Socioeconomic status, including whether the vendor is a woman owned, minority owned, blind or service disabled veteran owned; and 5) Resumes for key personnel that would work on the contract for individuals with knowledge and experience relevant to the requirement described in the draft Statement of Work (Resumes are not counted as part of the 5 page limit for capabilities statements).
