- Synopsis: This is a request for information (RFI). It is not a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to: (1) Obtain information on the manufacturing capability and capacity to produce these products. Background: The Centers for Disease Control and Prevention (CDC) is tasked with the design and construction services for the installation of a modular office building at the San Juan Campus. The modular office building will need to be easily reconfigured for the changing program needs and requirements. Requirement: CDC is seeking information from the manufacturing community pertaining to modular buildings. 1. The company will be required to have design and construction experience in the Caribbean. 2. The company must be a single source supplier for all materials and services. The company will be required to design, transport the modular building overseas, and provide a turnkey installation including connection to local utilities. The company will also need to provide furniture packages, electrical, light fixtures, plumbing, and HVAC for the modular building. 3. Modular buildings will need to be flexible, easily constructed in multiple phases over multiple years, and must be easy to relocate. The modular units must be able to stack up to three stories high without any additional structural requirements. 4. The company shall have the capability to design and construct a building at our San Juan Campus, a 2,000 sqft. modular office building within 120 days after the Notice to Proceed. 5. The modular building shall be capable of being built on pavers rather than a slab due to the existing site conditions. Submission Instructions: Respondents (manufacturer/distributor) are asked to provide only the most pertinent information, data, and materials necessary to adequately convey a declaration of capability in line with this notice. Respondents (manufacturer/distributor) are asked to provide a cover page that contains the following information: 1. All Point of Contact information (email addresses, phone numbers, and mailing address) 2. DUNS number. 3. All relevant NAICS Codes. 4. Identification as a large or small business. If small, indicate the classification. 5. Identification of products on Federal Supply Schedules, if any, and the relevant schedule numbers. 6. Identify if company is able to provide a turnkey solution and able to design, transport overseas, and construct the modular building. 7. Any additional information that could be beneficial to the CDC and was not requested. Proprietary information, if any, should be minimized and MUST BE CLEARLY MARKED. To aid CDC, please segregate and clearly mark proprietary information. Please be advised that all submissions become the property of the Government and will not be returned. This REQUEST FOR INFORMATION is in accordance with FAR 52.215-3 Request for Information or Solicitation for Planning Purposes (Oct 1997), as such, any information received will be for the purpose of planning ONLY. Responses to this REQUEST FOR INFORMATION notice, along with a firm's capability statement referencing " Request for Information - Modular Office Building" may be submitted electronically via email to the point of contact listed NO LATER THAN 11:00 AM EST, 4/17/2015.
