- Statement of Work Red Team Stairs and Sidewalk 544-15-3-5089-0312 1. REQUIREMENT: Provide and install stairs & sidewalk from Emergency Department roadway to ramp leading to building 103 in the area shown on the accompanying sketch and details. 2. BACKGROUND: The stairs are needed to help deter individuals from circumventing the ramp leading to building 103 by jumping over the handrail. 3. PERIOD OF PERFORMANCE: Working normal business hours, jobsite installation shall be completed within thirty days of start, (not counting fabrication time before arriving). Please note letter (i) below at the asterisk. The product and the installation will be warranted for 1 year after acceptance. 4. PLACE OF PERFORMANCE: Handicap ramp to building 103, near the smoking shelter. 5. PERFORMANCE REQUIREMENTS: Cut-out of retaining wall without over-cuts using wet-cut method. 48" wide expanded steel stair treads and landing assembled into a staircase, set in concrete or securely fastened to concrete, 12" deep treads, maximum 6" rise per step, self-closing gate at the beginning point of the 48" x 48" expanded steel landing, travel direction of the gate to prevent wheelchair entry into the landing as shown on the sketch. Nominal 3 ½" thick x 48" wide concrete sidewalk, maximum slope 1:20 extending from the bottom of the stairs to the curb, finished with a light broom finish across the sidewalk. Handrails on the landing and steps will be 1 ½" square steel tubing to meet code and painted exterior gloss black. A 4" tall toe kick-plate will be installed on the landing area. Gauge, thickness and quality of materials used will withstand constant use and withstand loads imposed by pedestrian traffic. Finished product will be free of sharp edges. A sketch with notes has been included in the package. a.Vendor will follow all OSHA requirements during installation. b.Vendor will clean up daily before leaving the worksite. c.Smoking is not permitted in any building except the smoking shelter or near building entrances. d.The vendor will provide and maintain at least one on-site a fully charged ABC fire extinguisher, minimum 10 pound capacity any time work is being performed. e.Stairs, landing and handrails must be fabricated before arrival. On-site fabrication must be minimal. Bolting is preferred, if minor welding is required, welding shields must be used to protect the public. f.Vendor arranges a work schedule with us so we can make the appropriate notifications a minimum of five working days in advance of arrival. g.Ambulance traffic must take priority and the roadway must remain passable at all times. h.Any removed debris or topsoil will be disposed of daily by the vendor, off-site. Piling of debris for removal later is not permitted. i.Cuts to the retaining wall will be wet cut only and without over-cut. Elevation of the cut-out must match the stair landing platform elevation surface once finished. ** Due to nearly continuous pedestrian traffic on the existing ramp, this is recommended to be done on a weekend or after-hours to avoid stopping and starting to allow pedestrians to pass. j.Provide a smooth, same level transition between the existing concrete ramp and the stair landing. Leaving the exposed cut brick is not permitted. k.The existing ramp will remain functional during the installation. Any openings made must be protected by rigid barricades and those barricades will be maintained until the project is complete. l.Areas excavated will be filled in with topsoil to meet concrete grade, topsoil raked smooth then grass seed applied and mulched with straw. m.Tools and equipment will not be left un-attended or left overnight. n.Vendor will maintain a barricaded construction area and any debris that escapes the area must be cleaned up immediately. o.All steel surfaces must be without any sharp edges, prepped, prime painted then finish painted with 2 coats of gloss black exterior industrial enamel paint. 6. INVOICES: a. Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor and submitted through Tungsten Network (formerly known as OB10) http://www.tungsten-network.com/us/en/. A properly prepared invoice shall contain: "Invoice Number and Date "Contractor's Name and Address "Accurate Purchase Order Number "Supply or Service provided "Period Supply or Service Provided "Total Amount Due b. Please begin submitting your electronic invoices through the Tungsten Network for payment processing, free of charge. c. TERMINATION FOR CONVENIENCE: Subject to a 14 day advance notice, the Government reserves the right to terminate this contract for convenience if the need for this service changes or is no longer required. 8. RECORDS MANAGEMENT LANGUAGE FOR CONTRACTS: The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: a.Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. b.Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. c.Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. d.Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. e.Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. f.The Government Agency owns the rights to all data/records produced as part of this contract. g.The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. h.Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. i.No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.
