Description

COMBINED SYNOPSIS/SOLICITATION - GENERAL INFORMATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation in its entirety; quotes are being requested and a written solicitation will not be issued. This procurement shall be conducted under FAR Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures, and Veteran Affairs Acquisition Regulations (VAAR) as supplemented with additional information in this notice. The award shall be based on the Offeror's ability to provide the specific supplies and services stated in the solicitation. Pursuant to FAR Part 13.303, Blanket Purchase Agreements (BPA), it is the intent of the Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 22 to establish a Single Award BPA, for the Brand Name or Equal reagents and consumables found in the attachments. This BPA shall have a base period of one (1) year and include two (2) one (1) year options. The awarded Offeror agrees to the following terms of the BPA exclusively with the VISN facilities listed in the attachment. However, as requirements change, facilities within VISN 22 may be added or deleted by supplemental agreement between the Government and the Contractor. Solicitation number VA 262-15-Q-0692 is issued as a "Brand Name or Equal" Request for Quotation (RFQ). This is not a small business set-aside; the North American Industrial Classification System (NAICS) code for this requirement is 334516, and size standard 500 employees. Due to time constraints, responses to this RFQ shall be due on Thursday, April 16, 1300 PST. Quotes must be submitted electronically via email to Gabe Caganap, Contracting Specialist at philip.caganap2@va.gov. Offeror's are advised to review and monitor the FBO website for the most up to date information, including all amendments. Correspondence or questions may be directed to Gabe Caganap at philip.caganap2@va.gov, with "VA262-15-Q-0692" in the subject line, within forty-eight (48) hours of the posting of this solicitation. Telephone inquiries will not be honored. Inquiries after the allotted period may not receive a response due to the time constraints of the procurement. SEE ATTACHMENTS FOR FULL SOLICITATION DETAILS. CONTRACT CLAUSES AND PROVISIONS The solicitation document and incorporated provisions and clauses are in effect through Federal Acquisition Circular 2005-78 effective December 10, 2014, respectively.
