- Representation and Certifications to fill out only if they are not completed at SAM.gov. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The Government intends to acquire a commercial item using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13. The Centers for Disease Control and Prevention contemplates award to a small business for purchase of gift cards which shall be given out to participants of various studies. The purchase order will be awarded as a blanket purchase agreement, with purchase of gift cards being charged on credit card. The purchase order will be utilized to set purchasing limits, to authorize users to make the calls, and for other contractual agreements. Approximately 10,000 cards are to be purchased over the course of a year, in amounts of $50, $25, $10, and $5; the cards may be VISA, Walmart, Starbucks, etc., depending on the geographical area for each purchase and the stores available. Money should be returned to CDC if the cards are not utilized (if we purchase 600, for example, and only use 590, the amounts funded on the 10 unused cards would be refunded). This procurement is a total small business set-aside. The CLINS should read as follows: Clin 1 Gift Card @.$50.00 ea Price each: ________________ Clin 2 Gift Card @$25.00 ea. Price each: ________________ Clin 3 Gift Card @$10.00 ea. Price each: ________________ Clin 4 Gift Card @$ 5.00 ea. Price each: ________________ This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-81 (April 10 2015). The NAICS Code for this procurement is 445110 and small business size standard is $32.5M. The offeror shall state in their offer their size status for this procurement. Vendors shall be registered at SAM.gov and their Representations and Certifications shall be complete. If you have not completed Representations and Certifications at SAM.gov, please fill out the attached Representations and Certifications form attached. All responsible sources may submit an offer which shall be considered by the agency. Quotes should be e-mailed to the purchasing agent at lml3@cdc.gov before close of business (4:30 p.m. ET) on April 21, 2015. Any questions should be addressed to the purchasing agent.
