The Department of Veterans Affairs, James A Haley VA Healthcare System 13000 Bruce B Downs Blvd Tampa FL 33612 intends to purchase the following (RFQ) Request for Quote for Brand name or Equal listed below: This is a Small Business procurement. All quotes must be received by the contracting officer via e-mail at Camille.Alderman@va.gov: entitled NUCLEAR MEDICINE ISOTOPE TRACKING SYSTEM Quote. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number, VA248-15-Q-0999 is issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective September 16, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. STATEMENT OF NEED NUCLEAR MEDICINE ISOTOPE TRACKING SYSTEM GENERAL: The Nuclear Medicine Service at the James A. Haley Veterans' Hospital (VHA) and Clinics 13000 Bruce B. Downs Blvd Tampa, Florida 33612 has a requirement for an Isotope Tracking System. Responses should reflect Brand Name or Equal, technically acceptable, new items only. No partial quotes. BACKGROUND: At this time, the VHA facilities are not permitted to keep paper logs containing sensitive patient information. We are currently using daily log sheets that are scanned and shredded weekly. The records are necessary for compliance with Nuclear Regulatory Commission (NRC) and National Health Physics Program (NHPP) statutes for the handling and administration of radioactive material. SCOPE: A firm fixed price purchase order will be issued as a result of this RFQ. The scope of this requirement involves hardware, software, installation, and on-site training to be provided to James A. Haley Nuclear Medicine Service only, on a one-time basis. Technical phone support, software updates, and hardware warranty to be provided to the James A. Haley Nuclear Medicine Service only, for a period no shorter than one calendar year. SPECIFIC TASKS: Provide technical onsite support for installation of the software to the Dose Calibrator in the Nuclear Medicine Hot Lab. Provide one day onsite applications training to staff and technical support provided by phone for one calendar year after completion. Software will record and track patient doses and any required Quality Assurance (QC) data that is initiated. Task 1. Provide, install and configure Nuclear Medicine Isotope Tracking System hardware/software, Barcode Readers and printers. Installation to include one day of in-house training for each system to be provided (including travel cost) to staff at agreed upon date and time organized by the Nuclear Medicine Service. The vendor staff will check in with the Biomedical Service on arrival. The Biomedical Service point of contact is Kevin Knotts, 813-972-2000 extension 7073. He will direct the vendor staff to log in to the RepTrax system and obtain vendor identification badges in Logistics prior to performing the installations. Access to the installation sites will be coordinated by the Nuclear Medicine Service. The point of contact is Alan Smith, 813-972-2000 extension 6674. Task 2. Provide and install Dose Calibrator Interfaces which shall include RS-232 (serial port communication) Output option and Dose Calibrator Cable. Task 3. Provide and install Nuclear Medicine Isotope Tracking System HL7 (Health Level 7) Interface implementation and DICOM (Digital Imaging and Communications in Medicine) Worklist Interface in collaboration with Biomedical staff. Configure backup to network drive in collaboration with Biomedical/OI&T. Task 4. Provide and install Nuclear Medicine Isotope Tracking System Desktop Hardware which shall include all the following as a minimum requirement: - Intel Core i3 system or newer - 4GB RAM (Random Access Memory) or more - 250 GB (gigabyte) Hard Drive - 20" Flat Panel LCD (Liquid-Crystal Display) Monitor or larger - Sound card - Speakers - DVD (Digital Video Disk) Drive - Windows 7 Professional 64 bit operating system installed - Keyboard - Mouse - 2 GB (gigabyte) USB (Universal Serial Bus) Flash Drive - 32 GB (gigabyte) USB (Universal Serial Bus) Flash Drive - 6' USB (Universal Serial Bus) cable - Printer - Surge Protector - 1 year parts and labor warranty or longer Task 5. Provide and install Dymo Twin Turbo Label Printers and Software Driver to include 4 rolls of label paper that is compatible with printer. PERFORMANCE MONITORING: All items have been delivered and appropriate training has been conducted. Installed equipment is working properly. Staff will be able to verbalize and demonstrate how to acquire data and generate reports from the system. OTHER PERTINENT INFORMATION OR SPECIAL CHARACTERISTICS: This software will allow for compliance for radiation safety audits from The Joint Commission, the National Health Physics Program and the Nuclear Regulatory Commission. Identification of Possible Follow-on Work - software Agreement to include Technical Support and updates after first year may need to be maintained after initial purchase. PLACE OF PERFORMANCE: Nuclear Medicine Hot Labs James A. Haley Veterans' Hospital. DELIVERY SCHEDULE: All items to be delivered and installed within 60 days of purchase order processing and order placement. FOB destination pricing. WARRANTY OF EQUIPMENT: Contractor must provide manufacturer(s) warranty on all provided equipment.
