- This is a sources sought announcement only and is not a request for proposals or quotes. The Department of Veterans Affairs Veterans Integrated Service Network (VISN) 4 is seeking to determine availability of SDVOSB or VOSB, and SB (small business) sources with the capability to provide Cryostat and Stainer Services for the VISN4 Lebanon VAMC. The following statement of work is offered for information only: A. GENERAL GUIDANCE EquipmentModelSerialPMI Slide StainerBond III32107006640-0027 Cryostat1900UV32430520066640-9082 CryostatCM 195019426640-0017 A.Title of Project: Silver Package Service Contract and Preventive Maintenance Inspection on above mentioned list of equipment. B.Scope of Work: In Accordance with the following specifications: 1)Service shall be provided in accordance with the Silver Service package listed in Vendor's Schedule A of service contract packages, which includes all labor, travel, parts (not including consumables), and 24/7 access to hardware and software technical phone support. Vendor shall provide all personnel, repair parts (except consumables), tools, test equipment, service manuals, computer software and schematics necessary to affect on-site repairs on all of the identified equipment during normal business hours, 8:00 AM - 5:00 PM Monday through Friday except holidays. Preventive Maintenance visits will be scheduled at the mutual convenience of the contractor and the Government. 2)The Vendor's service representative (VSR) shall contact Biomedical Engineering (717-821-3989) to schedule a Preventive Maintenance (PM) inspection or prior to responding to a service call. VSR shall acknowledge receipt of a service call within four (4) hours, and make plans to dispatch a field service engineer to affect on-site repairs of equipment listed in paragraph A. Equipment repaired by the VSR or loaned to the Medical Center by the vendor shall be calibrated and safety tested by the VSR before being placed into service. VSR shall not deviate from this protocol without the prior approval of Biomedical Engineering representative. 3)The vendor's service representative shall present the field service report to a representative of Biomedical Engineering for an authorized signature, leaving one legible copy for inclusion in the individual equipment's Historical Maintenance Record. Vendor shall also provide Biomedical Engineering with a legible copy of the calibration reports; to include numerical values of measurements taken. Failure to provide Biomedical Engineering with the field service and calibration reports within five (5) business days of equipment service and PM will result in non-payment of invoices submitted for this service. 4)Scheduled preventive maintenance shall include, but not be limited to, electrical safety testing, lubrication, adjustments, calibration, testing and replacement of faulty parts and parts which are likely to fail at no additional charge, returning the instruments or equipment to the operating conditions specified by the Manufacturer, NFPA 99 and VA MP-3 guidelines. A copy of the Preventive Maintenance Inspection procedure (including Qualitative and Quantitative tests) used by the vendor service representative for each item listed on the contract, shall be made available upon request. All work shall be performed by competent, experienced personnel qualified to work on the specified equipment in accordance with good manufacturing practices. Vendor shall provide factory training certificates/competencies for all technicians assigned to service the specified equipment, in accordance with JCAHO Standards. Such certificates/competencies shall be submitted to Biomedical Engineering within 30 days from date of contract award. Failure to provide required documentation will result in non-payment of invoices for this service. 5)The scheduled preventive maintenance shall be as follows: one (1) visit per year as per Biomedical Department request Vendor shall have the option of providing a scheduled preventive maintenance service at the same time as an emergency service call with the approval of the Biomedical Engineering representative. Firms with interest and capability to respond to this requirement should develop a short narrative including relevant experience and other information indicating and evidencing capability to successfully meet the aforementioned requirements.
