Description

AF Form 158 Item Unique Identification Checklist (IUID) Solicitation This is a solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being conducted as a Request for Proposal (Solicitaion No.) FA8571-15-R-0004. This solicitation is being solicited Under Other than Full and Open Competition IAW FAR 6.302-1, "Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirement" and will be a firm-fixed price Indefinite Delivery Indefinite Quantity award with a base + four option year periods. Requirment is being Sole Sourced to: Teradyne Incorportate (CAGE 23350) DBA: Assembly Test Division 700 Riverpard Dr. North Reading MA 01864-2634 Rationale for and Sole Source Justification information will be posted NLT 14 days after award. Price(s) should be FOB Destination (Robins AFB, GA 31098). In addition to price and delivery, responses should include your company's CAGE code, DUNS number, and tazpayer ID number. The applicable NAICS code for this requirement is: 334515. See attachment (Solicitation) for requirement specifics. All applicable clauses included in the solicitation are current through FAR FAC 2005-80 effective 2 Mar 2015 & DFARS DPN Notice 2150406 effective 6 April 2015) Questions should be emailed to Wendell Crittenden ( wendell.crittenden.2@us.af.mil ) no later than 10 May 2015. The anticipated award date is 15 July 2015, please do not request award status prior to this date.
