(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)The solicitation number is: VA259-15-Q-0354, and it is issued as a request for quotation (RFQ) (iii)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80. (iv)This solicitation is set-aside for Small Business. NAICS: 334510. (v)Contract Line Items: ItemDescriptionQty 0001Base Year. POP Begin 1 May 2015. POP End 30 Apr 2016. See below for product description. 30 bx (vi)Description of requirements for the items to be acquired (Statement of Work): a.ITEM DESCRIPTION: 100003331 Kit, Surface Electrode, EnSite Velocity. 5 kits per box. Item must be exact match. See below for Justification IAW FAR 13.106-1. b.This will be a Not-To-Exceed (NTE) order. There is no initial delivery. This amount is an estimated amount based on historic usage and is not guaranteed. The contractor is only due the amount for Services/Supplies actually provided. c.An "Authorized Government Caller" will be established and will have the authority to schedule specific delivery times (via telephone or email) for any quantity, as long as the orders do not exceed the dollar and quantity threshold and specified timeframe within the order. d.Orders must be delivered within 20 days ARO. DEPARTMENT OF VETERANS AFFAIRS Justification and Approval (J&A) For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 19, Salt Lake City VA Medical Center 660-15-3-060-0880. Estimated Dollar value per year: [REDACTED] 2.Nature and/or Description of the Action Being Processed: This procurement is for EnSite Electrode Kits in accordance with FAR 13.5 Test Program for Certain Commercial Items and specifically FAR 13.501 Special Documentation Requirements, where acquisitions conducted under Simplified Acquisition Procedures are exempt from the requirements of FAR Part 6, but still require a justification using the format of FAR 6.303-2. 3.Description of Supplies/Services Required to Meet the Agency's Needs: Required Items: EnSite Velocity Surface Electrode Kit #10000331 30bx annually (5each per box) 4.Statutory Authority Permitting Other than Full and Open Competition: FAR13.5 Test Program: The authority for applying the Commercial Test Program of FAR 13.5 is (Section 4202 of the Clinger-Cohen Act of 1996) and is implemented by for restricting competition on this procurement via FAR 13.106-1(b)(2). 5.Demonstration of Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): The VA Salt Lake City uses the EnSite system for cardiac mapping. To create the model, patches (called EnSite surface electrode patches) are applied in several places on the body. An electrical signal is transmitted between the patches, and catheters within the heart sense the signal. The physician then sweeps the catheter across the heart chamber to outline the structures and relay the signals to the computer system which generates the 3-D model. The diagram from the EnSite helps the physician guide the ablation catheter to the point in the heart where treatment is needed. The products needed are the Electrodes and catheters needed to work with the EnSite Velocity System that is already in place at the VASLCHCS. 6.Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: This requirement will be competed on FBO. 7.Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: The determination that the cost will be considered fair and reasonable will be made at the time of award. 8.Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: In researching online no other sources were found for this product. No sources found on and FSS or other government contract. 9.Any Other Facts Supporting the Use of Other than Full and Open Competition: N/A 10.Listing of Sources that Expressed, in Writing, an Interest in the Acquisition: N/A 11.A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: The VA Salt Lake City uses the EnSite system for cardiac mapping.
