This is a Request for Information and Sources Sought Notice for Market Research purposes only. THIS IS NOT A REQUEST FOR PROPOSAL, QUOTATION OR BID. NO SOLICITATION IS CURRENTLY AVAILABLE. The United States Property & Fiscal Office (USPFO) for Connecticut is seeking information regarding capability and availability of potential small business sources to provide Occupational Health Medical Monitoring support for the Connecticut Army National Guard (CTARNG) Occupational Health Department. All required services will be conducted in the Physician's Office. Connecticut ARNG requires licensed Physicians in the State of Connecticut, EXCEPT, Fairfield County due to the location of the CTARNG worksites. Physicians are defined as individuals possessing a degree in medicine or osteopathy and licensed by a State, Commonwealth, territory, or jurisdiction in which to practice medicine. The potential requirement is for Physicians interested and capable of providing qualified Health Care Providers (HCPs) to support the Occupational Health Department of the Connecticut Army National Guard to conduct baseline medical exams as well as follow up exams as needed upon the hiring and request of the employee or the Occupational Health Department. Medical exams are also required on exposures during the course of employment to hazardous materials. There is not a set amount of patients that will be seen during the duration and life of this contract. The number of patients is dependent on the number of new hires in the fiscal year, budgeting, possible deployments and sequestration. All services will be performed at the selected Physicians' Offices. The USPFO for Connecticut is seeking qualified Small Business (SB) Community Members [Small Business (SB), Small Disadvantaged Business (SDB), Section 8(a), Veteran Owned Small Business (VOSB), Service Disabled Veteran Owned Small Business (SDVOSB), Women Owned Small Business (WOSB), HUBZone Small Business, Small Business (SB) led teams, Small Business Partnerships and or Small Business Joint Ventures] that qualify under the North American Industry Classification System (NAICS) Codes: 621111 Offices of Physicians. The Small Business Size Standard is $11,000,000.00 This Request for Information and Sources Sought Notice is part of Market Research. Your responses are sought to identify potential sources that have the knowledge, skill, equipment, experience and past performance necessary to perform the aforementioned occupational health medical monitoring services. All interested, responsive and responsible contractors are encouraged to participate in this Market Research by submitting a response via e-mail to Frank Tantillo, Contract Specialist at frank.a.tantillo.mil@mail.mil no later than 1:00 PM EST April 29, 2015. Please limit responses to four (4) pages or less. This notice does not constitute a request for a formal proposal. This notice is provided as information to the market place and is an invitation for an expression of interest and brief demonstration of qualifications to perform and deliver the anticipated requirement.
