Description

FBO-DESCRIPTION This is a Full and Open Request for Quote (RFQ) for a requirement for a contractor to provide service for the Water Purification System. It will be solicited as a Combined Synopsis/Solicitation. The solicitation number is RFQ number VA258-15-Q-0128_1. The services are located at: Phoenix VA Healthcare System Research Lab 650 E. Indian School road Phoenix, AZ 85012 It is to procure a periodic maintenance service contract for one Water Purification System ROSLW1003 (s/n: 191951) that is being used in the Research Program of the Phoenix VA Health Care System (PVAHCS). The vendor is to accomplish scheduled and required maintenance on an automatic periodic schedule. The contract must include at Water Purification Services and component exchanges, in order to meet manufacturers suggested maintenance requirements. The service contract will include all parts and labor for maintenance of the Water Purification System. This unit provides laboratory grade water for all of the research laboratories located within Building 21. The water is utilized in experiments that include cells and tissues. Also, the purified water is used in various experimental analyses of samples obtained from human subjects in research trials that are conducted at the Medical Center. Furthermore, purified water is used in running and maintaining specialized pieces of scientific equipment that are located in the laboratories. It is of the utmost importance that water purity is maintained and that resources are available to immediately fix the unit when it is not working. The period of performance is for a base plus 4 years. The unit consists of a water softening unit, multiple activated charcoal filters, size exclusion filters (micron filters), mixed resin beds de-ionizing tanks, and a UV lamp sterilizing unit that all require periodic maintenance and/or replacement. The water purification system also requires periodic repairs of plumbing, gaskets, and various electrical components that run pumps that fill a storage reservoir and maintain water pressure through specialized pipes that carry the water to water faucets in the labs. Breakdowns of the unit are greatly minimized with the performance of preventative maintenance visits which should be included in the service contract. Any prospective contractors must be authorized/certified to provide the required service from the manufacturer and must show proof with any provided documents at the time of responding to this combined synopsis / solicitation. This solicitation closes April 22, 2015 no later than 11:00 a.m. CST. Responses should be based on the material contained in this RFQ. Responses will not be returned. Telephone responses will not be accepted. QUESTIONS: Questions and responses shall be submitted by email to Garland D.
