- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. Solicitation number VA249-15-Q-0436 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. This requirement is Small Business (SB) set aside. The NAICS is 541380 and the small business size standard is $15.0 Million. The government anticipates awarding a Firm Fixed Price award. The bases of award will be Lowest Price Quote. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforms to the solicitation requirements and will provide the best value to the government. Description of Services: The James H. Quillen (Mountain Home) VA Medical Center located at Corner of Lamont & Veterans Way, Mountain Home, Tennessee, 37684 has a requirement for Dialysis Water Testing at the James H. Quillen VA Medical Center. Submission of Quote: The Offeror shall submit their quote on company letterhead and shall include unit price, total, unit quantity, terms of any express warranty, unit price, overall total price, applicable shipping charges and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov. Complete copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. The offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The offeror shall provide past performance evidence. This assessment is based on the offeror's record of relevant and recent past performance information that pertains to services outlined in the solicitation requirements. See Attached: Statement of Work STATEMENT OF WORK The Contractor agrees to provide Laboratory services in accordance with the terms and conditions stated herein, to furnish to and at the Department of Veterans Affairs Medical Center, Mountain Home VA Healthcare System, Mountain Home, TN, the services and prices specified in the Section entitled Schedule of Supplies/Services of this contract. 1. INTRODUCTION AND SCOPE OF WORK a.The contractor shall perform certain environmental monitoring tests on water and dialysate specimens provided by the James H. Quillen VA Medical Center, Mountain Home, Tennessee. Those tests shall be performed in an environmental laboratory certified by the national Environmental laboratory Program (NELAP), or equivalent, to the standards detailed in ANSI/AAMI/ISO Publication 23500:2011 and the results shall be reported referenced to those standards. b.The tests performed shall be those listed in the Schedule of Services and the expectation is monthly with six to eight additional tests annually as needed based on conditions. 2. DIALYSIS WATER VERIFICATION AND MONITORING a.The quality of the dialysis water, as specified in 3.2 and 3.3. shall be verified by testing of specimens obtained by the VAMC Dialysis Unit staff and shipped to the Contractor's environmental laboratory using sampling/shipping containers provided by the Contractor in accordance with ANSI/AAMI/ISO Standard 11663-2009, ANSI/AAMI/ISO Standard 23500-2011, and ANSI/AAMI/ISO Standard 13959-2009, and no prior standards. b.Microbiological requirements - Reporting of Total viable microbial counts in dialysis water after testing using culturing techniques according to the above-specified standards shall be reported using a maximum allowable level of less than 100 CFU/ml, or lower if required by national legislation or regulations. The reporting action level shall be will be 50 % of the maximum allowable level. c.Reporting of Endotoxin content in dialysis water shall be in accordance with the limits set in the above-specified standards and shall be such that detection of less than the maximum allowable level of 0.25 EUml is possible, or lower if required by national legislation or regulations. Reporting sensitivity shall be such that an action level of 50 % of the maximum level can be identified and reported. d.Microbiology of dialysis water - Contractor shall provide acceptable specimen sampling and shipping containers such that Total viable counts (standard plate counts) shall be obtained using conventional microbiological assay procedures (pour plate, spread plate, membrane filter techniques) on the culture media specified in the above ANSI/AAMI/ISO standards. Membrane filtration is the preferred method for this test. The calibrated loop technique is not accepted. e.Culture media will be tryptone glucose extract agar (TGEA), Reasoners 2A {R2A), or other media that can be demonstrated to provide equivalent results, as per the specified ANSI/AAMI/ISO standards. Blood agar and chocolate agar shall not be used. Incubation temperatures of 17Cto 23 C and Incubation time of 168 h (7 d) will be used. Other incubation times and temperatures may be used If It can be demonstrated that they provide equivalent results. No method will give a total microbial count. f.The presence of endotoxins shall be determined by the Limulus amebocytelysate (LAL) test other test methods may be used if it can be demonstrated that they provide equivalent results. g.Also, it is important to refer to Section 5 (Page 6) of ANSI/AAMl/ISO Standard 11663; "Tests for compliance with microbiological requirements" and Section 4, starting on page 9, of ANSI/AAMl/ISO Standard 23500:2011;Summary of quality requirements of ISO 13959 and ISO 11663" 3. CONTRACT PERFORMANCE MONITORING a. Monitoring of Contractor's performance will be monitored. All testing supplies will be provided at no additional cost as well as shipping. The results will be made available to the VA within 72 hours.
