Description
Performance Work Statement Wage Determination •1. This is a combined synopsis/solicitation for commercial supply prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Solicitation Number is FA6648-15-T-0002. The North American Industry Classification System (NAICS) Code is 722515 " Snack and Nonalcoholic Beverage Bars", this acquisition is 100% set aside for small business concerns. Small business size standard shall not exceed $7.5 million in sales per year. This solicitation document with the incorporated provisions and documents are those in effect through Federal Acquisition Circular 2005-72. •2. The Department of the Air Force 482nd Homestead ARB, FL has a requirement to operate a Non-Appropriated Fund (NAF) Shake/Juice and Nutrition Bar concession inside the Sam Johnson Fitness Center located on Homestead Air Reserve Base in accordance with the attached Performance Work Statement (PWS). This contract is to provide a concession within the Sam Johnson Fitness Center for a base period to begin on or about 25 May 2015 through 24 May 2016 followed by four (4) possible extensions/option years. The contractor will pay the NAFI a $200.00 flat rate per month for the first year of the contract. Each of the following years a 2% increase will be applied for the right to operate a Shake/Juice and Nutrition Bar inside the Sam Johnson Fitness Center. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation provides the best value to the Government. Best Value will be determined according to the following factors which are of equal importance: •1. Variety of menu items. (How many items are available to customers) •2. Quality of ingredients used. (protein, supplements, boosters) •3. Price of product as sold to the customer. •4. A site visit shall be scheduled for 28 April 2015 at 1:00 pm Eastern Time at Homestead ARB, FL. Interested parties should send a request to Shamekia.Lee@us.af.mil NOT LATER THAN Friday, 24 April 2015 at 3:30 pm Eastern Time indicating the Full Names of the individuals who will be attending, their Date of Birth, and Driver's License Number with Issuing State. Your request for attendees should be submitted on company letterhead. This information will be used to obtain base access passes and safeguarded IAW instructions under the Privacy Act of 1974. •5. Offerors are required to be registered in Systems for Awards Management (SAM) to receive an award. The solicitation will be made available only on FedBizOps website. Paper copies will not be available. No telephone calls or written requests for this solicitation package will be accepted. Once the solicitation is posted, it is the interested parties' responsibility to review the site frequently for any updates/amendments to the solicitation. •6. Proposals are due in the Homestead ARB, FL contracting office no later than 4:00 P.M. local time on 4 May 2015 via standard mail delivery or e-mail. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
