INSTRUCTION TO BIDDERS REP & CENT VENDOR REGISTRATION PAY SCHEDULE SCA WAGE EXECUTIVE PAY FAR FLOW DOWN DRAFT SUBCONTRACT   SCOPE OF WORK THIS IS A SUBCONTRACTING OPPORTUNITY with Fluor Federal Solutions LLC   NAICS Code for proposed work effort is 621210 Small Business size limit is: $7.5   Gainesville Job Corps Center GJCC-316-004 DENTIST Provides dental services and program supervision including, Oral examinations, emergency treatment and routine dental care Minimum Qualifications: Active, unrestricted license to practice in the State of Florida; Current DEA registration Scope of Dental Services: 1. The services of the Professional will be made available to the Center at the Center's medical facilities in Gainesville, Florida as follows: 2. Such services will be provided not to exceed 9 hours per week, Monday through Friday as mutually agreed upon, to include emergency on-call and telephone consultation services. Holidays are excluded.   3. Some routine care may be performed at the Professional's private office. The professional and the Center director will mutually agree upon specific hours of operation and dental procedures 4. The services to be provided include, but are not limited to: a. Provide a routine dental examination of the American Dental Association Classification, Type 11; b. Provide a written diagnosis and treatment plan for each enrollee. c. Provide necessary preventive dental health procedures, e.g., tooth brushing instruction, which will promote the oral hygiene of all enrollees. FLUOR PROPRIETARY INFORMATION   d. Provide appropriate referral system to ensure the receipt of special care required. e. Provide coverage by a qualified substitute during the absence of the Professional. f. Provide for twenty-four (24) hour emergency coverage. g. Provide a diagnosis and treatment plan for each student, on the basis of which priorities for the receipt of dental care will be established. 5. Monitoring of the total Center health Program to include: a. Advise the Center Director and the Health Services Supervisor on all dental and health related issues. b. Provide assistance in developing the Dental Health program c. Ensure the maintenance and confidentiality of all dental records d. Provide assistance in the development and implantation of the dental education component of the Health Education Program. e. Identification and utilization of the most appropriate and least expensive sources for obtaining dental pharmaceutical supplies. f. Provide for a comprehensive set of "standing orders" updated annually SUBMIT: 1. Current Resume 2. Active License 3. DEA Registration 4. Attachment 1 with salary requirements. Pricing Option: Dental Services for this Center allow for: One (1) Dental Assistant   Such services will be provided not to exceed 12 hours per week The Dentist may employ a Dental Assistant. Should that option be exercised, the Dentist (employer) will be required to comply with hourly rates in accordance with the Service Contract Act (copy attached). Occupation Code 12020 Dental Assistant shows the minimum allowable "base rate" plus, page 7 of the SCA schedule shows the required "benefits", that must be included. The Dentist must also certify that the employee is covered under a Workman's Compensation Program. GJCC is a federally‐funded residential educational facility serving roughly 300 area youth in vocational, academic, and employment training & education services. Fluor Federal Solutions, LLC. (Fluor) invites you to provide your individual Firm-Fixed-Price bid for this proposal, in accordance with the Instructions to Offerors. This is a Request for Proposal (RFP) and does not commit any funds for procurement. Proposals must be received no later than 3 :00 PM Eastern Time, 23 April 2015, in accordance with Instructions to Bidders.   Proposals shall be valid for (90) days.
