- Required Insurance Clauses and Provisions Evaluation Criteria Pricing Schedule Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is RFQP0700COB0150151 and it is issued as a Request for Quote (RFQ). The requirement will be solicited as a total small business set. The applicable North American Industry Classification System NAICS) code is 621111, Other Accounting Services and the size standard is $11.0 million. The Federal Bureau of Prisons, Central Office Business Office intend to establish a single award, indefinite delivery requirements firm-fixed price type contract for the Mortality Review Services to be rendered for the Office of Quality Management, Health Services Division. The Mortality Reviewer is required to objectively review individual mortality cases of federal inmates that deceased while under the medical care of the Bureau of Prisons as required by Bureau of Prisons policy, Joint Commission, and ACA standards. The purpose of this review is to focus on the quality and appropriateness of medical care provided to each inmate prior to his or her death. A description of the services required is attached to this posting and labeled as Statement of Work. Also, attached to this posting are the Evaluation Criteria as well as the Pricing Schedule, Required Insurance and Clause and Provisions. Evaluation will be "go/no go" meaning ALL identified criteria must be met to be considered for award. Award will be based on lowest price offered to the Government among technically acceptable responses. A technically acceptable response is defined as one that meets all of the minimum technical requirements identified in the Statement of Work. This requirement is being solicited no set aside is being used. It is expected that the solicitation will be available on or about April 20, 2015 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fedbizopps.gov. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure the most up-to-date information about this acquisition is obtained. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. All contractors doing business with the Federal Government shall be registered in the System for Award Management database. Failure to satisfy this requirement will result in an immediate exclusion from any award consideration. The website for registration is www.sam.gov. In addition, interested parties should also be registered and complete the On-Line Representations and Certifications Application (ORCA). QUOTES SHALL BE SUBMITIED AS A FIRM-FIXED PRICE PER SESSION SHEET LINE ITEM. ALL EVENT SHEET LINE OPTIONS MUST BE PRICED. QUOTES MUST BE SUBMITTED ON THE PRICING SCHEDULE ATTACHED TO THIS SYNOPSIS/SOLICITATION. PROPOSALS NOT SUBMITTED USING THE PRICING SCHEDULE WILL NOT BE CONSIDERED. QUOTES WILL BE EVALUATED ON THE ABILITY TO MEET REQUIRMENTS AND ON PRICE. Interested parties are hereby notified that any contract awarded based on this solicitation will be of a non-personal nature. Faith-Based and Community- Based Organizations (AUG 2005)-can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible.
