Description
MRI Diffusion Calibration Phantoms The Supply/services to be rendered here under shall consist of four (4) MRI Diffusion Calibration Phantoms (Custom/Proton). These items are to be used in the Research Service Line of the Michael E. DeBakey VA Medical Center located at 2002 Holcombe Blvd in Houston, Texas 77030. The contractor shall provide all necessary tools, interfaces, equipment/hardware, software per Statement of Work (SOW) dated May 1, 2015. Instruction to Offerors Guidance: This sources sought notice is not a Request for Proposal (RFP). No solicitation is being issued at this time. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the government to a particular acquisition approach. Any information provided to the Government as a result of this sources sought notice is voluntary. Responses will not be returned. No entitlement to payment or direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses or the Government's use of such information. This Sources Sought Notice is for planning purposes only. INSTRUCTIONS FOR SUBMISSION After review of the above information, interested parties may submit a response in an electronic format only (via email) to Leslie Robinson, Contract Specialist leslie.robinson@va.gov. Respondents should include as part of their submission: a.Indicate whether or not you can support the procurement requirement based on the items identified in the Statement of Work b.Any questions that you have reference this Sources Sought Notice c.Company point of contact name, telephone number and email address. d.Socioeconomic status/Business Size (Other Than Small Business, Small Business etc.) e.Provide DUNS number, CAGE code, and indicate whether or not you are registered in the System for Award Management (SAM) database (Website: www.sam.gov). (Registration in SAM is a requirement to do business with the Federal Government). Statement of Work Four (4) MRI Diffusion Calibration Phantoms (custom/Proton) Michael E. DeBakey VA Medical Center Houston, Texas 77030 April 20, 2015 1.PURPOSE: The purpose is to procure the following items: four (4) MRI Diffusion Calibration Phantoms (Custom/Proton) for the Michael E. DeBakey VA Medical Center located at 2002 Holcombe Blvd, Houston, Texas 77030.The contractor shall provide all resources necessary to accomplish the deliverables described in the statement of work (SOW), except as may otherwise be specified. 2.SCOPE: (Specifications/Salient Characteristics) 2.1.The suggested manufacturer is Psychology Software Tools OR Equal is preferred. 2.2.MRI Diffusion Calibration Phantom (Custom/Proton) (in Qtr. 2) 1 Each 2.3. MRI Diffusion Calibration Phantom (Custom/Proton) (in Qtr. 3) 2 Each 2.4. MRI Diffusion Calibration Phantom (Custom/Proton) (in Qtr. 4) 1 Each 3.BACKGROUND: Research Investigator will be utilizing the above named items in support of her VA Funded Research Project. 4.PERIOD OF PERFORMANCE: May 1, 2015 - September 30, 2015 5.TYPE OF CONTRACT: Firm-Fixed-Price. 6.PLACE OF PERFORMANCE: Michael E. DeBakey VA Medical Center 2002 Holcombe Blvd. Bldg. 100, Room 2B-120C Houston, TX 77030 7. GENERAL REQUIREMENTS 8.1. For every task, the contractor shall identify in writing all necessary subtasks (if any). 8.2. All written deliverables will be phrased in layperson language. Statistical and other technical terminology will not be used without providing a glossary of terms. 8. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in section 14 of this SOW. DELIVERABLES: "Four (4) MRI Diffusion Calibration Phantoms (Custom/Proton) 9. SCHEDULE FOR DELIVERABLES 9.1 The contractor shall complete the delivery date column in section 14 for each deliverable specified. 9.2. Unless otherwise specified, the number of draft copies and the number of final copies shall be the same. If for any reason the scheduled time for a deliverable cannot be met, the contractor is required to explain why (include the original deliverable due date) in writing to the CO, including a firm commitment of when the work shall be completed. This notice to the CO shall cite the reasons for the delay, and the impact on the overall project. The CO will then review the facts and issue a response in accordance with applicable regulations. 10. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. 11.GOVERNMENT RESPONSIBILITIES The government will provide controlled access to the areas and other resources if required to perform the services. The government will assign a primary POC, and to provide information and resources in a manner to maintain contract continuity. Additionally, this person will receive deliverables as a result of this contract. 12.SECURITY 12.1 CONTRACTOR PERSONNEL SECURITY All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Security and Investigations Center (07C). The level of background security investigation will be in accordance with VA Directive 0710 dated September 10, 2004 and is available at: http://www.va.gov/pubs/asp/edsdirec.asp (VA Handbook 0710, Appendix A, Tables 1 - 3). Appropriate Background Investigation (BI) forms will be provided upon contract (or task order) award, and are to be completed and returned to the VA Security and Investigations Center (07C) within 30 days for processing. Contractors will be notified by 07C when the BI has been completed and adjudicated. These requirements are applicable to all subcontractor personnel requiring the same access. If the security clearance investigation is not completed prior to the start date of the contract, the employee may work on the contract while the security clearance is being processed, but the contractor will be responsible for the actions of those individuals they provide to perform work for the VA. In the event that damage arises from work performed by contractor personnel, under the auspices of the contract, the contractor will be responsible for resources necessary to remedy the incident. The investigative history for contractor personnel working under this contract must be maintained in the databases of either the Office of Personnel Management (OPM) or the Defense Industrial Security Clearance Organization (DISCO). Should the contractor use a vendor other than OPM or Defense Security Service (DSS) to conduct investigations, the investigative company must be certified by OPM/DSS to conduct contractor investigations. 13. VA INFORMATION AND INFORMATION SYSTEM SECURITY/PRIVACY LANGUAGE FOR INCLUSION INTO CONTRACTS, AS APPROPRIATE 13.1 GENERAL Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. The storage facility will have controlled access, proper environmental controls, and reinforced concrete or steel beam construction that has been earthquake proofed. Access control to the VA information stored at this location will be stringently controlled and periodically tested. Locks and personnel will be used to control the off-site storage to prevent unauthorized access. 14.
