The purpose of this Sources Sought Announcement is for market research only to make appropriate acquisition decisions and to gain knowledge of potential qualified businesses interested and capable of performing the work. Interested contractors should respond to this notice by providing the following: a)Company name b)Address c)Point of contact d)Phone, fax, and e-mail of primary point of contact e)DUNS number f)Type of small business, if applicable, (e.g. Service Disabled Veteran Owned Small Business, Veteran-owned small business, 8(a), HUB-Zone, Women Owned Small Business, Small disadvantaged business, or Small Business) and g)A capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below: The Department of Veterans Affairs is seeking to provide for the Eastern Kansas Health Care System (EKHCS) metropolitan area as part of its Community Based Health Care for Homeless Veterans (HCHV) program. The EKHCS is located at 4101 South 4th Street, Leavenworth, KS 66048 and 2200 Gage Boulevard, Topeka, KS 66622 and services the following counties in Missouri: Buchanan, DeKalb, Gentry, Holt, Nodaway, Platte, and Worth and Kansas: Anderson, Atchison, Brown, Bourbon, Chase, Cherokee, Hay, Cloud, Coffey, Dickinson, Doniphan, Douglas, Franklin, Geary, Jefferson, Leavenworth, Lyon, Marshall, Morris, Nemaha, Neosho, Osage, Pottawatomie, Riley, Shawnee, Wabaunsee. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. Through the HCHV program, VA provides case management services to Veterans and facilitates their access to a broad range of medical, mental health, and rehabilitative services. The purpose of the solicitation will be to obtain proposals from contractors who can provide housing, care, treatment and rehabilitative services to homeless veterans, who may also be suffering from serious mental illness. These community-based treatment facilities should offer a safe and secure environment that supports their rehabilitation goals. The Contractor will be required to provide therapeutic and rehabilitative services, but will not be required to provide detoxification or other hospital level treatment - those services will be provided by the VA at VA facilities. Contracts will have a base term of one year, with options for four additional one-year terms exercisable in the discretion of the VA. The following additional information is provided: a.Contractor shall be expected to provide same day residential placement that enhances the stability of Veterans services by the HCHV. b.The contractor will ensure that participants are monitored 24 hours a day, 7 days a week to include all National, Religious, and Federal Holidays. Provide sufficient personnel to assure security, plan and coordinate all activities associated with providing sage and appropriate care. c.The contractor will also provide transportation for Veterans to and from the VA. All Offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) database found at https://www.sam.gov. Registration must include Representations and Certifications. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Business who respond to a solicitation on this project must be registered and verified with the Vet-Biz Registry located at http://VIP.vetbiz.gov. The VA anticipates issuing a solicitation for this requirement on or about April 30, 2015, with a closing date for proposals on or about May 29, 2015. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments.
