Description
Bins requiring pickup twice per month Bin locations COMBINED SOLICITATION/SYNOPSIS FOR COMMERCIAL SERVICES. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N68096-15-RC-09048 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 561990. Size Standards in Millions of dollars: $10.0 The US Naval Hospital Guam, Agana Heights, GU has a requirement for the following services: Shredding of Personally Identifiable Information (PII) CLIN 0001: Vendor shall provide a total of forty-eight (48) 100LB capacity bins. Bin contents shall be collected at different departments and floors of US Naval Hospital, Guam and its Branches. Branch Medical Clinic and Branch Industrial Hygiene/Audiology are located at Naval Station Guam. Forty-one (41) bins to be collected on the 30th of each month and seven (07) to be collected twice a month, once on the 15th and on the 30th (See bin distribution attached). Vendor shall shred all PII in accordance with Health Insurance Portability and Accountability Act (HIPPA) regulations and be certified with the National Association for Information Destruction (NAID). Vendor is required to submit certification documents with RFQ submission. Quantity: 12 Months Unit Price: _____ Total Price: _____ CLIN 0002: Annual Bulk Shredding, twenty thousand pounds (20,000 LBS). As required and when requested by US Naval Hospital, Guam, Vendor shall shred all PII in accordance with Health Insurance Portability and Accountability Act (HIPPA) regulations and be certified with the National Association for Information Destruction (NAID). The government will only consider firm, fixed price quotations. Any submissions that do not meet the minimum requirements shall not be considered. Contractor must be registered in SAM prior to award. The website address for SAM registration is www.sam.gov. QUOTATIONS DUE: 27 April 2015, 10:00AM Guam Time / 26 April 2015, 5:00PM Pacific Time. The submitter should confirm receipt of email submissions. The attached solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-75.
