- Description
- The QSRS is an expandable and multi-functional software platform to permit the collection, and analysis of specified patient safety and quality of care data. This system currently supports Web-based medical records abstraction at the CMS Clinical Data Abstraction Center (CDAC) for surveillance regarding adverse events that reach the patient. QSRS will replace, as well as expand on the functionality of, the current CMS/AHRQ Medicare Patient Safety Monitoring System (MPSMS) chart abstraction software. The current Contract HHSA290201200009C is to build and maintain the AHRQ Quality & Safety Review System ("QSRS") and subsequent versions of QSRS may expand on these basic functions to include surveillance of a wider range of quality issues. The sole source modification will add a new capability to the current QSRS software to facilitate implementation of QSRS at the local (hospital or hospital-system) level. As specified in the SOW, this will: 1) allow data entry to work on cases (abstraction of individual patient records) from systems other than the one operated by CMS at the CDAC; 2) allow and support the use of external data feeds for Admit Discharge Transfer (ADT) information, as well as subsequently loading and testing of the feed; 3) provide case management features as specified by AHRQ based on a subset of those currently present at the CDAC; and 4) provide the capability to link cases of mothers and cases of their newborn infants (link medical record numbers for the mother with the newborn infant(s)'s charts to facilitate an institution's review of obstetrical charts and outcomes). These capabilities will be necessary for independent optional use of QSRS outside of the CMS CDAC, which will support HHS initiatives currently in preparation in support of the Affordable Care Act, in particular the "Hospital Engagement Network 2.0" initiative underway by CMS (see https://www.fbo.gov/index?s=opportunity&mode=form&id=3d476468ff9ecad47b5c63a41d95b7db&tab=core&_cview=1). Edaptive is uniquely qualified to perform this software development task because they are currently completing the enhancement and implementation of the version of the QSRS software that will be used at the Clinical Data Abstraction Center (CDAC). As this work proceeds there may be changes to the software that would influence the implementation, completion and integration/release of the "local" version of QSRS. Edaptive is the only contractor that can perform this work in order to maintain the quality, validity and consistency of the enhancements within the QSRS. The use of another contractor would significantly increase the possibility of disruption to both the quality and validity of the system enhancements. Delaying the local version of QSRS until the CDAC version is in routine use is not an option as it would delay the development of the local version of QSRS to a timeframe beyond CMS's and HHS's needs associated with the "Hospital Engagement Network 2.0" program of the Partnership for Patients program, which is expected to run from August 2015 to August 2016. This notice of intent is not a request for competitive proposals. A determination by the Government as to whether or not to compete this proposed modification action based upon responses to this notice is solely within the discretion of the Government. The final Justification for Approval (J&A)/Justification for Other than Full and Open Competition (JOFOC) supporting this sole source action will publicly be made available as required after the contract modification award.
