- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested and a written Solicitation will not be issued. This is a Total Small Business Set-Aside requirement only qualified offers may submit bids. Solicitation VA-249-15-Q-0433 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. NAICS Code is 561210 and size standard is $38.5M. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. Contract Type: The government anticipates awarding Firm Fixed Price purchase order. Award will be made to the lowest price quote which conforms to the requirements within this solicitation. Service Contract Act (SCA) Wage and Determination rules and regulations are applicable to this request and can be accessed at http://www.wdol.gov In order to be considered for a government award, the firm must be registered in SAM @ www.sam.gov. Description of Services: This requirement is for the VAMC Memphis. The Department of Veterans Affairs, Network Contracting Office located in Murfreesboro, Tennessee intends to issue a firm fixed priced purchase order. The contractor shall provide services for the removal of existing damaged concrete from around manway, removal of manway, and installation of new composite, water tight manway that supports up to heavy traffic, and install new concrete in accordance with Statement of Work below. Quote Submission: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. All prospective bidders must include appropriate references which must include all applicable company information. The offeror shall provide sufficient evidence that they possess adequate resources, capability, experience, responsibility and integrity to meet the technical capabilities to comply with the requirements of the resulting contract. The offeror must provide past performance evidence. Past performance will be evaluated on an "acceptable" or "unacceptable" basis using the ratings in the table below based on the answers to the above capabilities questionnaire. The past performance evaluation results is an assessment of the offeror's probability of meeting the minimum past performance solicitation requirements. This assessment is based on the offeror's record of relevant and recent past performance information that pertain to the products and/or services outlined in the solicitation requirements. Past performance information may be obtained through other sources known to the VA and the Federal Government. In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past. Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability "unknown" shall be considered "acceptable." A Site Visit will be made available to interested parties at the Memphis VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104 on April 28, 2015 at 10:00AM CST. In order to attend the site visit, interested parties must submit the names of individuals, as well as point of contact information to Matthew.Cox@va.gov no later than 8:00AM CST, April 24, 2015. Submissions after the "no later than" date will not be accepted. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. STATEMENT OF WORK PART A - GENERAL INFORMATION A.1 INTRODUCTION: Remove damaged, surrounding concrete & diesel fuel oil, underground tank manhole replacement. A.2 BACKGROUND: The diesel fuel oil tank manway is a covered opening in the ground that provides access to the fuel oil tank sensors, and access to the fuel oil tanks if needed. A.3 QUALITY ASSURANCE: Equipment and materials installed shall be compatible in all respects with existing items so that the result will be a safe, complete and fully operational system which conforms to contract requirements and in which no item is subject to conditions beyond its design capabilities. All elements shall be designed with the latest published version of applicable codes. A.4 PLACE OF PERFORMANCE: VAMC Memphis- Department of Veterans Affairs Medical Center, 1030 Jefferson Ave., Memphis, TN 38104. A.5 PERIOD OF PERFORMANCE: Date of Award - Until Completion. A.6 TRAVEL: The Contractor is responsible for all of their travel, accommodations, meals, per diem, rental cars, etc. A.7 WORK HOURS: 8 am - 4 pm A.8 SCOPE OF WORK: Vendor shall remove existing damaged concrete and replace manway for tank #6. A. TANK MANHOLE ENCLOSURE: Vendor shall install new Tank Manhole Enclosure; OPW Pomeco 39" composite watertight manhole 39CD-WT10 or equal. 1. In traffic areas, enclosure must be designed to withstand traffic loads (H-20 wheel loading, 14 500 kg, (32,000 lb)). 2. Access to Manhole Enclosure: Composite material manhole frames and covers, rated for traffic, minimum opening to match existing. B. CONCRETE: 1. Re-enforcement - The type, amount, and locations of steel reinforcement shall be as shown in the specifications. C. SELECT SUB-BASE (WHERE REQUIRED): 1. Materials meeting other gradations than that noted will be acceptable whenever the gradations are within a tolerance of three to five percent, plus or minus, of the single gradation established by the job mix formula, or as recommended by the geotechnical engineer and approved by the Resident Engineer. 3. Sub-base material shall produce a compacted, dense graded course, meeting the density requirement specified herein. D. FORMS: 1. Use wood forms that are straight and suitable in cross section, depth, and strength to resist springing during depositing and consolidating the concrete, for the work involved. 2. Do not use forms if they vary from a straight line more than 1/8 inch (3 mm) in any ten foot (3000 mm) long section, in either a horizontal or vertical direction. 3. Wood forms should be at least 2 inches (50 mm) thick (nominal). Wood forms shall also be free from warp, twist, loose knots, splits, or other defects. Use approved flexible or curved forms for forming radii. E. CONCRETE CURING MATERIALS: Not used F. EXPANSION JOINT FILLERS: Material shall conform to ASTM D1751-04. PART B EXECUTION B.1. SETTING FORMS A. BASE SUPPORT: 1. Compact the base material under the forms true to grade so that, when set, they will be uniformly supported for their entire length at the grade as shown. 2. Correct imperfections or variations in the base material grade by cutting or filling and compacting. B. FORM SETTING: 1. Set forms sufficiently in advance of the placing of the concrete to permit the performance and approval of all operations required with and adjacent to the form lines. 2. Do not remove forms until removal will not result in damaged concrete or at such time to facilitate finishing. 3. Make necessary corrections to forms immediately before placing concrete. 4. When any form has been disturbed or any subgrade or sub-base has become unstable, reset and recheck the form before placing concrete. B.2 PLACING REINFORCEMENT: A. Reinforcement shall be free from dirt, oil, rust, scale or other substances that prevent the bonding of the concrete to the reinforcement. All reinforcement shall be supported for proper placement within the concrete section. B. Before the concrete is placed, the Resident Engineer shall approve the reinforcement placement, which shall be accurately and securely fastened in place with suitable supports and ties. The type, amount, and position of the reinforcement shall be as shown on the Drawings. B.3 PLACING CONCRETE GENERAL A.Obtain approval of the Resident Engineer before placing concrete. B.Remove debris and other foreign material from between the forms before placing concrete. C.Before the concrete is placed, uniformly moisten the subgrade, base, or subbase appropriately, avoiding puddles of water. D.Convey concrete from mixer to final place of deposit by a method which will prevent segregation or loss of ingredients. Deposit concrete so that it requires as little handling as possible. E.While being placed, spade or vibrate and compact the concrete with suitable tools to prevent the formation of voids or honeycomb pockets. Vibrate concrete well against forms and along joints. Over vibration or manipulation causing segregation will not be permitted. Place concrete continuously between joints without bulkheads. B.4 CONCRETE FINISHING - GENERAL: A.The sequence of operations, unless otherwise indicated, shall be as follows: 1.Consolidating, floating, straight-edging, troweling, texturing, and edging of joints. 2.Maintain finishing equipment and tools in a clean and approved condition. B.5 CONCRETE FINISHING: A. After the surface has been struck off and screeded to the proper elevation, provide a smooth dense float finish, free from depressions or irregularities. B. Carefully finish all slab edges with an edger having a radius as shown in the Drawings. B.6 FORM REMOVAL: A. Forms shall remain in place at least 12 hours after the concrete has been placed. Remove forms without injuring the concrete. B. Do not use bars or heavy tools against the concrete in removing the forms. Promptly repair any concrete found defective after form removal. B.7 PROTECTION: The contractor shall protect the concrete against all damage prior to final acceptance by the Government. Remove concrete containing excessive cracking, fractures, spalling, or other defects and reconstruct the entire section between regularly scheduled joints, when directed by the Resident Engineer, and at no additional cost to the Government. Exclude traffic from vehicular pavement until the concrete is at least seven days old, or for a longer period of time if so directed by the Resident Engineer. Vendor shall remove all debris, rubbish, and excess material from work site upon completion of work. Employees must have a valid state or government identification. All contractor/vendor personnel shall display a VAMC Memphis issued identification badge while performing work on any VAMC property. Smoking is not prohibited on property except in designated areas. Possession of weapons is prohibited (in vehicle or on person). Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. Contractor employees (crew) when on VAMC Memphis grounds; shall not bring visitors into the facility; shall not bring into the facility, any form of weapons or contraband; shall not bring in any alcohol or drugs or be under the influence of alcohol/drugs; shall conduct themselves in a professional and polite manner at all times; shall not cause any disturbances in the facility, are subject to all the rules and regulations of the VAMC Memphis. Contractor personnel shall present a neat appearance and be easily recognized. Contractor shall furnish and require every on-site company employee to wear a company shirt and safety shoes. All company workers while on duty shall be identified by uniform shirt or blouse indicating the company logo. The print should be large enough to be easily read. In addition all company staff assigned to work at the VAMC Memphis. Access to VA Sensitive information is not required; therefore no security clauses are required. NOTE: The Contractor employees shall not be considered government employees for any purpose under this contract. C.3 WORKING CONDITIONS/SAFETY: The contractor/vendor shall at all times, ensure that all work provided complies with all local, State, and Federal rules and regulations pertaining to work place safety. The contractor shall take all proper safety and health precautions to protect the work, the workers, the public and the property. The contractor/vendor is solely responsible for working conditions on the jobsite, including safety of all persons and property during performance of the work, and for compliance with applicable OSHA regulations. Work shall be done in such a manner so as to create a safe working situation and be of little disruption for all staff, patients and visitors throughout the VAMC Memphis.
