- Enclosure 5 - Past Performance Information Form Enclosure 4 - FAR and HSAM Clauses Enclosure 3 - Wage Determination Enclosure 2 - SOW Enclosure 1 - Schedule of Prices/Costs This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and FAR Subpart 13.5 procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (A) The solicitation number is HSCG84-15-Q-B17082 and is issued as a Request for Quotation (RFQ). (B) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. Service Contract Labor Standards Wage Determination Number 1996-0061, revision 28, dated 12/22/2014 is applicable and is attached as Enclosure Three. (C) This acquisition is issued as unrestricted. The associated NAICS code is 721110. (D) List CLINS, item descriptions, quantities and units of measure: SEE ENCLOSURE ONE FOR SCHEDULE OF SERVICES AND UNIT PRICES. (E) Description/Statement of Work/Specification SEE ENCLOSURE TWO FOR SOW. (F) Date(s) and place(s) of delivery and acceptance. SEE SOW FOR REQUIRED DATES AND PLACE OF PERFORMANCE (G) FAR Provision 52.212-1, Instructions to Offerors-Commercial, is incorporated by reference and applies to this acquisition. SUBMISSION REQUIREMENTS Quote Content An Offeror shall submit a signed and dated quote on company letterhead stationery that contains, at a minimum, the following sections and information: (1) Cover Page/Letter shall include the following at a minimum: (a) Request for Quote (RFQ) number (b) Name, Address, Telephone Number, and DUNS Number of the Offeror (i.e., the company name, etc) (c) Name, Title, Phone Number, and E-mail Address of an Authorized Official of the Offeror (d) A statement specifying the following: "We agree with all the terms, conditions, clauses, and provisions included in the Request For Quote (RFQ)." (2) Technical Capability and Understanding Section: Shall consist of the offeror's technical approach to the task, addressing that they at least meet all of the requirements set forth in the SOW. The Technical Section shall be limited to 3 pages; additional pages will not be reviewed or evaluated. Each "page" is one single sided 8.5" x 11" sheet. Each page shall have at least one inch margins on all sides and Times New Roman font with a 12 point size type or larger. (3) Past Performance Section: Offerors shall furnish the information listed in Enclosure five (5) for at least 2 different contracts, that are ongoing or completed within the last three (3) years, are relevant to this requirement (i.e., the same or similar requirement) and are held with government (federal, state, local), private or commercial entities. Like or similar requirements under this solicitation is defined as those providing the same or similar services or supplies to a government, private, commercial entity. Note: The Government may use past performance information obtained from any source to evaluate past performance. All information obtained will be used to determine the offeror's ability to perform the contract successfully. (4) Price Section: The Schedule of Services and Unit Prices filled out in its entirety. See Enclosure one (1). (H) Evaluation: FAR PROVISION 52.212-2: Evaluation - Commercial Items (OCT 2014). The Government will award a contract(s) resulting from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to award one contract to one vendor who can provide all the rooms under one contract. In the event that one vendor cannot provide all the rooms or it is not in the best interest to award to just one vendor based on price or technical matters, the Government may consider awarding this to more than one vendor in order to meet the Government's requirements. If more than one vendor is selected, it will be based on the best interest and what is most advantageous to the Government, price and other factors considered as listed above. The following factors shall be used to evaluate offers: (1) Technical Capability and Understanding, (2) Past Performance, and (3) Price. All evaluation factors other than price, when combined, are significantly more important than price; however, price will become more important as the differences between the other factors become less. PLEASE SEE ENCLOSURE FOUR (4) FOR COMPLETE EVALUATION DETAILS. (I) Offerors are required to include a completed copy of FAR Provision 52.212-3, Offeror Representations and Certifications-Commercial Items (MAR 2015), with their offer. Offerors may complete FAR 52.212-3 online at SAM: www.sam.gov. An offeror must state in their offer if they completed FAR 52.212-3 online and that is it up-to-date and valid. (J) FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items (DEC 2014), is incorporated by reference and applies to this acquisition. (K) FAR Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (APR 2015) is incorporated by reference and applies to this acquisition. NOTICE FOR FILING AGENCY PROTESTS United States Coast Guard Ombudsman Program It is the policy of the United States Coast Guard (USCG) to issue solicitations and make contract awards in a fair and timely manner. The Ombudsman Program for Agency Protests (OPAP) was established to investigate agency protest issues and resolve them without expensive and time-consuming litigation. OPAP is an independent reviewing authority that is empowered to grant a prevailing protester essentially the same relief as the Government Accountability Office (GAO). Interested parties are encouraged to seek resolution of their concerns within the USCG as an Alternative Dispute Resolution (ADR) forum rather than filing a protest with the GAO or some external forum. Interested parties may seek resolution of their concerns informally or opt to file a formal agency protest. Informal Forum with the Ombudsman. Interested parties who believe a specific USCG procurement is unfair or otherwise defective should first direct their concerns to the applicable Contracting Officer. If the Contracting Officer is unable to satisfy their concerns, interested parties are encouraged to contact the U.S. Coast Guard Ombudsman for Agency Protests. Under this informal process the agency is not required to suspend contract award performance. Use of an informal forum does not suspend any time requirement for filing a protest with the agency or other forum. In order to ensure a timely response, interested parties should provide the following information to the Ombudsman: solicitation/contract number, contracting office, Contracting Officer, and solicitation closing date (if applicable). Formal Agency Protest with the Ombudsman. Prior to submitting a formal agency protest, protesters must first use their best efforts to resolve their concerns with the Contracting Officer through open and frank discussions. If the protester's concerns are unresolved, an Independent Review is available by the Ombudsman. The protester may file a formal agency protest to either the Contracting Officer or as an alternative to that, the Ombudsman under the OPAP program. Contract award or performance will be suspended during the protest period unless contract award or performance is justified, in writing, for reasons of unusual and compelling urgency or is determined in writing to be in the best interest of the Government. The agency's goal is to resolve protests in less than 35 calendar days from the date of filing. Protests shall include the information set forth in FAR 33.103(d) (2). If the protester fails to submit the required information, resolution of the protest may be delayed or the protest may be dismissed. To be timely protests must be filed within the period specified in FAR 33.103(e). Formal protests filed under the OPAP program shall be submitted electronically to OPAP@uscg.mil and the Contracting Officer or by hand delivery to the Contracting Officer.
