- STATEMENT OF WORK (INCLUDES DELIVERY SCHEDULE) 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, streamlined procedures for evaluation and solicitation for commercial items-as supplemented with additional information included in this notice. This acquisition is unrestricted. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This document incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-76. All responsible sources in accordance with the NAICS code 519190 may submit a quote. 2. RFQ number DOL-OPS-15-Q-00044 applies and is issued as a Request for Quotation (RFQ). Offerors must be registered in the System for Award Management at www.sam.gov, in order to be considered for award. 3. For exact text and wording of clauses and provisions, please see http://farsite.hill.af.mil. The Government contemplates an award of a Firm Fixed Price, resulting from this RFQ. 4. The North American Industry Classification System (NAICS) code is 519190 and the business size standard is $25.5 million. All responsible sources may submit a quotation NLT the closing date of 25th May, 2015 11am, EST will be considered by the Agency. 5. Description of supplies. All responsible Contractors shall provide a quote for the following CLINs and the attached Statement of Work (SOW). CLIN 0001- Base Year - Contractor shall provide supervision, expertise, labor, materials and supplies to furnish Criteria files of active U.S. businesses, defined by OSHA, in accordance with the SOW and the Delivery Schedule. Period of Performance: June 1st, 2015 - May 31st, 2016. CLIN 0002- Option Year One-Contractor shall provide supervision, expertise, labor, materials and supplies to furnish Criteria files of active U.S. businesses, defined by OSHA, in accordance with the SOW and the Delivery Schedule. Period of Performance: June 1st, 2016 - May 31st, 2017. CLIN 0003- Option Year Two-Contractor shall provide supervision, expertise, labor, materials and supplies to furnish Criteria files of active U.S. businesses, defined by OSHA, in accordance with the SOW and the Delivery Schedule. Period of Performance: June 1st, 2017 - May 31st, 2018. CLIN 0004- Option Year Three-Contractor shall provide supervision, expertise, labor, materials and supplies to furnish Criteria files of active U.S. businesses, defined by OSHA, in accordance with the SOW and the Delivery Schedule. Period of Performance: June 1st, 2018 - May 31st, 2019. CLIN 0005- Option Year Four-Contractor shall provide supervision, expertise, labor, materials and supplies to furnish Criteria files of active U.S. businesses, defined by OSHA, in accordance with the SOW and the Delivery Schedule. Period of Performance: June 1st, 2019 - May 31st, 2020. 6. Award will be made on the basis of lowest price technically acceptable proposal (LPTA) IAW 12.203 and elements of 15.101-2. Award will be made to the responsible offeror whose offer represents the lowest priced offer that is technically acceptable. The Government intends to evaluate offers and award without discussion. Offeror shall submit all information with offer as required in 52.212-1. To verify the data is technically acceptable, the Department requests a sample file for review. Please provide a file with all of the data elements required by the SOW for the following industries in the indicated states: 1) SIC 3444; one or more employees; FL and MO 2) SIC 5181; ten or more employees; NJ and MD 3) NAICS 327211; one or more employees; TX and CO Please provide the file in Excel, csv or txt delimited format. If the file cannot be delivered in one of these formats, please indicate a format prior to delivery. Offers shall be received no later than May 25th, 2015 11AM EST via email to the attention of Ms. Stacy Conaway, CPPB, at Conaway.stacy@dol.gov
