Description

The Department of Veterans Affairs Network Contracting Office (NCO 6) intends to negotiate a sole source contract with the Institute for Healthcare Improvement to provide a learning collaborative with three VISN 6 teams located at Salem VA Medical Center 1970 Roanoke Blvd, Salem, VA 24153; Hampton VA Medical Center 100 Emancipation Dr. Hampton, VA 23667 and Charlotte Health Care Center VA Outpatient Clinic, 8601 University East Drive Charlotte, NC 28213 to meet patients medical and behavior needs. The VISN 6 teams will fully use the collaborative framework to design a new model of care for Veterans with medical and behavioral comorbidities that improves their health and experience of care at lower cost. This will require redefining the composition and roles of primary care and mental health and building highly functional multidisciplinary teams that are fully equipped to address the medical and behavioral care needs of their Veterans population in their local setting. Period of Performance will end February 28, 2016. Justification for this sole source procurement is in accordance with FAR 13.106-1(b)(2) soliciting from a single source. This notice of intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be accepted. However, interested parties may identify their interest and capability to respond to this requirement no later than Noon, April 30, 2015, local time. A determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. NAICS Code 611430.
