- Draper Cabin Map North Rock Creek Cabin Map Moe Cabin Map Inspection Report D Inspection Report C Inspection Report B Inspection Report A RFQ This is a COMBINED SYNOPSIS / SOLICITATION for commercial items prepared in accordance with the format in the Federal Acquisition Regulations (FAR) Part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation issued as Request for Quote (RFQ) AG-82D7-S-15-0009. FAR clauses and provisions enclosed within this RFQ are those in effect through Federal Acquisition Circular (FAC) 2005-80 and are available in full text through Internet access at http://www.acquisition.gov/far. The U.S. Forest Service, White River National Forest, has a requirement to remove asbestos containing materials from three cabins and one abandoned recreational vehicle per the attached Statement of Work. This acquisition is a 100% small business set-aside; the Forest Service encourages the participation of veteran, disadvantaged and women owned business enterprises. The North American Industry Classification System (NAICS) code for this project is 562910 with a size standard of $19 million. All responsible small business concerns may submit a quotation which shall be considered by the Forest Service. Quotes shall be delivered by the specified time electronically to jtobin@fs.fed.us using the attached SF18, Request for Quotation. Alternatively, they may be delivered to USFS/GMUG, Attn: Jay Tobin, 2250 Highway 50, Delta, CO 81416. Facsimile proposals will not be accepted. Contractors who have been found "guilty" of committing asbestos enforcement violations by the Colorado Department of Health and Environment during the past five years shall be ineligible for award. Contractors who have pending cases under investigation shall submit an explanation of the circumstances of said case for consideration by the Government as part of its contractor responsibility determination. Award will be made on the basis of lowest priced, technically acceptable offer. Offerors shall provide copies of licenses, training certificates, and summary of relevant experience for personnel performing these services. Offerors must be registered at the System for Award Management ( www.sam.gov ) to be eligible for award.
