- Statement of Work (SOW) Service Agreement Carestream DRX-Evolution SF-1449 (RFQ #CLA-15-Q-0008 THIS SOLICITAION IS ISSUED AS FULL AND OPEN COMPETITON. Service Agreement (Care Plus Full Coverage) for the Carestream DRX Evolution x-ray machine, Room A, at the Claremore Indian Hospital, Claremore, Oklahoma, 74017. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. FAR Par 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994) (Public Law 103-355) & FAR 37.4, Non-personal Health Care Services Contract (10 U.S.C. 2304 and 41 U.S.C.253). This announcement constitutes the only solicitation; Request for Quotes (RFQ) are being requested and a written solicitation will not be issued. The Claremore Indian Hospital (IHS) intends to award a firm fixed-price, non-personal service type contract, commercial item; purchase order in response to solicitation number CLA-15-Q-0008. This acquisition is a full and open competition. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-79. The associated North American Industry Classification System code (NAIC) code is 811219 and the Small Business size standard is $20.5 million. This acquisition will not be subject to Service Contract Labor Standards. Period of performance: This contract will be from June 1, 2015 through May 31, 2016, with Four Option Years. *Subject to Availability of Next Year's Funds, FAR 52.232-19 incorporated by reference NOTE: A new order will be issued for each option year. Offers will be accepted electronically no later than May 11, 2015, 4:30pm CST. After this date proposals will not be accepted. Questions after this date will not be accepted. Questions will be answered April 30, 2015 by 4:30pm CST. Contractor will need a DUNS number & TIN number and be registered with www.SAM.gov. PURPOSE OF CONTRACT: See attached Statement of Work. Location is at the Claremore Indian Hospital, Claremore, Oklahoma 74017. Basis for award shall be Lowest Price Technically Acceptable "LPTA" proposal package. 1. Past performance evaluation shall be made on past performance of service of similar equipment. 2. All proprietary software upgrades shall be included in service agreement. 3. Submit three (3) references from healthcare facilities located in the North East Oklahoma Area that contractor had performed preventative maintenance and services. References shall include: Healthcare facility name, contact name, contract number and telephone number 4. Proposal should provide a 95% or greater percentile of guaranteed equipment up-time status operating according to manufacturer design specifications, based upon average 6 month period. See SOW, page 1, section 3. Detailed Description of the Technical Requirements. The unit pricing must be an all-inclusive cost (to include but not be limited to transportation, food, lodging, per diem, federal, state and local taxes), plus all other costs pertinent to the performance of this contract. Utilize your most competitive and reasonable rates. PLEASE COMPLETE THE ATTACHED PROPOSAL SCHEDULE AND SUBMIT. INCLUDE PRICE PER MONTH AND OVERALL PRICE PER LINE ITEM FOR BASE YEAR AND OPTION YEAR WITH AN OVERALL GRAND TOTAL. Attached are the Federal Acquisition Regulations (FAR) and the Health & Human Services Acquisition Regulation (HHSAR) clauses that are applicable including the following: FAR 52.212-1 - Instructions to Offerors-Commercial Items (Apr 2014) Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number CLA-15-Q-0008 2. Closing date of May 11, 2015 at 4:30pm CST 3. Name, address, telephone number of the offeror and email address of the contact person. 4. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include documents, if necessary; 5. Terms of any express warranty 6. Price and any discount terms 7. "Remit to" address, if different than mailing address 8. A completed copy of the representations and certifications at FAR 52.212-3 9. Acknowledgement of Solicitation Amendments. 10. A statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
